Job Description

The Nurse Navigator provides ongoing support and care to patients in the Lung Diagnostic Assessment Program (LDAP), Colorectal Diagnostic Assessment Program (CDAP) and the Malignant Pleural Effusion (MPE) outpatient clinics at OTMH. This nurse ensures that patients receive the resources and education they need to manage their condition. They also play a critical role in coordinating care between our different specialties and ensuring that our patients receive the best possible care.

These Diagnostic Assessment Programs are designed to evaluate individuals who may have symptoms or concerns related to lung cancer (LDAP) or for patients that have been diagnosed with colon or rectal cancer (CDAP). Within these clinics we work closely with our patients to develop a personalized treatment plan that addresses their unique needs and concerns. Our goal is to provide a comprehensive and timely streamlined approach to complete all required investigations within the guidelines of Cancer Care Ontario (CCO) and reduce patient wait times and worries.

The Oakville MPE Clinic provides patients with a central point of contact from initial investigations and therapeutic interventions to ongoing follow up of symptoms related to their malignant pleural effusions. We provide innovative and novel intervention techniques including tunneled pleural catheters and ultrasound guided thoracentesis to help with symptom management. Our goal is to provide patients with symptom relief and focus on ensuring quality of life is met including a focus on maintaining time in their home environment.

  • Assist in triaging referrals

  • Assist with procedures in the clinic

  • Ensure any follow up diagnostic tests are complete

  • Provide education to patients, families, and significant others; act as an information resource

  • Collect and report data from patients on health history, medications, prior treatments, barriers etc.

  • Monitor patient compliance with patient treatment plan

  • Assist patient with referrals to outside resources and care coordination as needed

  • Follow established departmental policies/procedures, objectives, safety, and infection control standards

  • Engage with community partners to ensure seamless transitions and consolidated care

Qualifications

  • Bachelor of Science in Nursing degree, Masters preferred

  • Current member in good standing with the Ontario College of Nurses

  • At least 5 years of acute care experience

  • Prior experience in a DAP preferred

  • Excellent organizational, interpersonal and problem solving skills

  • Self-directed and highly motivated

  • Ability to cultivate positive working relationships with internal and external stakeholders

  • Satisfactory employment and attendance record

  • Excellent written and verbal communication skills with the ability to explain and provide necessary information to patients, family members, physician and/or any stakeholders

  • Effective and efficient time management and organization skills with the ability to manage competing demands

  • Excellent leadership, collaboration, decision making, problem recognition, problem solving, and critical thinking skills

  • Diplomacy and public relations skills required

  • Computer literacy required, database management preferred

  • Demonstrated and ongoing commitment to continuing professional development

  • Evidence of program design and resource development skills

Additional Information

Hours: Monday to Friday; Days 8 hour shifts (Hours are subject to change based on operational needs)

Salary: $39.07 to $56.00 per hour
