About the role

The Concurrent Disorders Assertive Community Treatment (ACT) program is a new innovative partnership initiative between Unity Health Toronto (St. Michael’s Site) and Toronto Public Health. The program relies on an inter-professional team to provide community-based assertive community treatment services (home visits, community visits) to individuals living with serious mental illness and substance use disorders in downtown Toronto.

This role will be responsible for the delivery of high quality care to clients based in the community and offer supports to those experiencing serious mental illness and substance use disorders. This includes the screening of clients, coordinating necessary support services, monitoring mental health and substance use related concerns, and keeping accurate records of client care.

DUTIES AND RESPONSIBILITIES

  • Meets all new clients and acquaints them with the various options and/or programs available internally/externally
  • Provides support to the clients in identifying personal goals and lifestyle changes
  • Provides counseling for clients and ensures continuity in service
  • Acts as a liaison with the community agencies to ensure appropriate client referrals
  • Networks with community agencies and acts as a client advocate
  • Demonstrates excellent ability to develop relationships across the hospital and community agencies
  • Facilitation of groups
  • Completes Provincial Assessment Tools when required
  • Has experience and knowledge of community case management in addictions and mental health
  • Liaises with primary care providers to ensure services for clients

QUALIFICATIONS INCLUDE:

  • Two year college diploma in Social Service Work, Psychology, Mental Health and Addictions, Human Service Counselling, Addictions Counselling.

  • Certification as a CSW preferred.

  • Minimum 2 years’ experience working in the mental health and addictions field and familiarity with the addictions continuum and other sector partners which may be a support to the client population is preferred

  • Experience working as mental health and/or addictions caseworker preferred

  • Current CPR certification

  • Sound clinical knowledge of withdrawal symptoms and appropriate interventions.

  • Experience with Motivational Interviewing, Community Reinforcement Approach, Dialectical Behavioural Therapy preferred

  • Excellent written & verbal communication skills

  • Ability to work independently as well as in a team framework

  • Must be comfortable working with clients in diverse external locations and have skills to engage family members of clients

  • Ability to work flexible hours as a requirement of the positions, including weekends and evenings

  • Good attendance record

  • Willingness to attend required training
