Job Description Job Summary: As a member of the interdisciplinary team, the Transitional Support Worker (TSW) is responsible for assisting youth in their successful integration into the community from a secure setting. This is accomplished through rehearsing and instructing youth in the practical application of the skills and coping strategies acquired during their stay at SAYC. The TSW completes pass and pass programs along with supervising youth in the community while they practice their skills. The TSW connects youth to community agencies and placements to prepare youth for discharge.
This is a 40hr work week position that requires shift work with alternating weekends.
This position is covered by the Ontario Public Service Employees Union on behalf of its local 213. Duties & Responsibilities:
- Provide care to youth through the therapeutic milieu, attend plan of cares, prepare pass programs and passes for youth. Supervise youth within SAYC along with supervising youth on a pass. Connect youth to community based programs and recreational/leisure passes. Prepare youth for discharge, attend placements with youth in assisting them in integrating back into the community or less restrictive environment. Involved in working with the multi-disciplinary team in planning care for youth in a secure environment.
- Prevent or manage aggressive behavior. Engage in case management duties for youth. Supervise youth within the unit or during visits. Attend Plan of cares, clinical meetings, team meetings and Community agency meetings.
- Prepare written reports, incident reports, Daily Observations, communication book entries, Movement logs. Clinical Rounds. Plans of Care updates. Contact community agencies for information to support youth at SAYC.
- Process Admissions and Discharges.
- Record and administer approved medications, unit searches.
Qualifications:
- Requires completion of Community College Diploma or equivalent. **
- Some practical or position related experience required.
- Bilingualism (English and French) considered an asset.
- Board Record Check - for working with Vulnerable Populations
- Must have G Class License
