Rate: $50.08 - $64.20 Hourly

Unit Summary

Hamilton Health Sciences Pharmacy department is committed to Pharmacy Excellence. The Pharmacist promotes evidence-based/cost effective drug therapy, provides drug information, participates in quality improvement activities and promotes clinically relevant research. Pharmacists will be at times required to mentor Pharmacy residents, students, and interns. Being an integral member of the Pharmacy team, the Pharmacist has a comprehensive understanding of the drug distribution system and participates in processes that ensure safe medication practices (e.g. computerized order entry, medication reconciliation). In addition, Pharmacists are required to participate in Pharmacy related activities (e.g. committees, review of order sets) and duties as assigned.

Position Summary

The Pharmacist is a member of the multidisciplinary healthcare team that provides pharmaceutical care to patients at Hamilton Health Sciences. The Pharmacist reviews and assesses medication therapies and works with the patient and the healthcare team directly to optimize medication use and minimize medication-related problems to achieve desired patient outcomes. This work may be performed on individual clinical units or may be across multiple units to support the medication system needs of the site.

Schedule Work Hours

Days, Evenings, Nights, Weekends, Statutory Holidays, Over-Night On-Call

This position is temporary until October 2026

Qualifications

  • Bachelor of Science in Pharmacy required. PharmD is an asset.
  • Current registration with the Ontario College of Pharmacists required – Part A of Register Previous hospital experience in unit dose and/or IV admixtures preferred Hospital residency program or equivalent an asset Demonstrated ability to work independently and as a member of a team Demonstrated ability to prioritize a consistent and varied workload Ability to direct and teach staff.
  • Demonstrated written and verbal communication skills required, in order to facilitate effective communication with patients, peers, and other health care providers.
  • Demonstrated ability to identify, prevent and resolve drug related problems.
  • Active membership in the Canadian Society of Hospital Pharmacists (CSHP) is an asset.
  • Good understanding of change management; must be able to identify change needs and proactively act on those needs.
  • Leadership, problem solving, organization skills required.
  • Effective negotiation and conflict management resolution skills required.
  • Excellent attendance and discipline free record required.
  • Hospital Pharmacy Residency preferred
  • Relevant ICU experience preferred

Proficiency in both Official Languages would be an asset.
