Organizational Background
The Greater Hamilton Health Network (GHHN) is one of the first Ontario Health Teams across the province, with a purpose of transforming healthcare in partnership with patients, families, care partners, primary care, local organizations and the community. A collaboration of local health and social service partners, the GHHN includes representation from more than 150 organizations. We are a dedicated group of professionals, organizations and people with lived experience working to co-design a patient centered health system, grounded in engagement, health equity and the local needs of the communities we serve across Hamilton, Haldimand, and Niagara Northwest.
True to its population health roots, the GHHN is especially focused on creating opportunities to integrate care across a range of services for patients with complex needs. Position Summary
The GHHN is looking for a collaborative leader who understands the primary care sector, appreciates the vision for a more integrated health care system, and works collaboratively and effectively with the providers of primary care in Hamilton, Haldimand and Niagara North West to build a more connected primary care network. This position will focus on strengthening relationships within and between the GHHN Primary Care Network and health and community stakeholders.
The successful candidate will be action-oriented with a focus on working in partnership with primary care providers to deliver positive change in the primary care sector – positive change for both patients and providers.
Working closely with Primary Care Network Leadership and Executive Councils, this role will drive change through local co-design. Evidence-based and Best-practice initiatives will be the foundation of the work that will be measured through the lens of the quintuple aim: Patient experience, health outcomes, provider experience, value and health equity.
What We are Looking For

Masters degree in Business, Health Administration program or related field an asset

Experience in Primary Care Sector required
-
3-5 years of practical and related experience
-
Project Management Experience/Certificates and asset
-
Experience managing staff
-
Experience launching successful change initiatives in the health care community
-
Stakeholder engagement, project planning, management and implementation experience required
-
Change Management & Relationship Management experience required
-
Experience with successful Digital Health/IT initiatives an asset
-
Demonstrated knowledge and application of quality improvement principles (e.g. Lean Methodology) as asset
-
Excellent written, oral Communication skills and Interpersonal Skills

Excellent Time Management, meeting timelines

Ability to work as a team, professional and courteous manner

Exercise good judgement, initiative, with ability to multi-task

Knowledge of Ontario Health sector and the healthcare transformation to Ontario Health and Ontario Health Teams considered an asset

Your Mission The Manager of Primary Care will be responsible for the execution of the following initiatives:

The growth and sustainability of the GHHN Primary Care Network
-
Administrative leadership supporting the Co-Chairs of the Primary Care Network Leadership Council and Executive Council
-
Initiatives focused on Primary Care Access and Attachment, screening, early intervention, and digital health enablers for primary care
-
Integrated care and population health management initiatives impacting primary care
-
Embedding health equity principles in all co-design
-
Leading from an equity, diversity and inclusivity lens
-
Embeds in all related projects the shared vision, guiding principles and strategy of the, GHHN including principles of Health Equity, Diversity and Inclusion
-
Guided by the OHT Building Blocks, supports the implementation of projects and work plan deliverables for the GHHN
-
Initiate and plan assigned strategic projects including the development of scope, goals and objectives
-
Develop work plans, project schedules, cost estimates and budgets as required
-
Engage key partners to ensure project activities are executed based on plans and standards, within the specific scope, budget and schedule
-
Execute, track and monitor progress of the initiatives while distributing the appropriate information to the appropriate committee structures
-
Identify improvement opportunities and escalate key issues/risks; propose and execute solutions as approved that achieve quality work and process improvements
-
Problem solve to result in innovative and creative solutions
-
Develop and present material for review and information using excellent written, verbal and graphical communications skills with the ability to communicate technical aspects effectively
-
Working with the cross-functional team to support the measurement of OHT (GHHN) performance indicators to ensure regular measurement and monitoring of OHT (GHHN) initiatives
-
Support the implementation of data collection methods and analysis
-
Provide management and HR oversight for direct reports
Working Conditions
The work environment is currently virtual (subject to change) with a minimum of 2 in-person meetings/month in the city of Hamilton. In-person team meetings are mandatory and can be called at the discretion of the Executive Director.
For virtual work we require the individual to have a private, dedicated workspace from home, free from distraction. A laptop will be provided.
Hours of Work Typical working hours are 8:30am to 4:30pm (Eastern Time), Monday to Friday, with some exception for community engagements that may occur virtually or in-person outside these hours. It should be noted that primary care physician hours for meetings may fall outside the hours of 8:30am and 4:30pm and it is required this role meet virtually with Primary care physicians on a regular basis.
