The Director of Resident Care (DRC) has a key leadership role, with oversight of clinical services delivered by Rotary Hospice Stratford Perth (RHSP) and management of resident care within the eight-bed hospice home.
The DRC facilitates a highly supportive culture and professional practice environment conducive to philosophies of compassionate and caring hospice palliative care, specifically as related to end-of-life care. The DRC will provide supervision and coordination of the care team which includes regulated and unregulated providers and support staff, promoting a culture of care, and a diverse workplace that fosters inclusion, dignity, and respect.
Responsibilities Resident Care & Delivery
- Able to identify and respond to emerging hospice palliative care modalities, research, and practices.
- Provides leadership and innovative thinking to foster resident-centred solutions to the provision of hospice palliative care in the current environment.
- Reviews and assesses residential hospice service and related service delivery.
- Works with Executive Director in the review of admission criteria respective to provincial and national standards
Clinical Care & Leadership
- Works with the resident and family, Hospice Medical Director, Hospice Physicians, and Interdisciplinary Team to develop the resident’s palliative care plan.
- Manages referrals, assessments, intake and waiting list in collaboration with the Clinical Resource Nurse
- Provides supervision and support to hospice clinical care staff.
- Ensures that best practice policies and procedures are in place for clinical operations, to include medications, medical supplies, food, equipment, etc.
- Coordination of staff schedules
- Participates in clinical staff interviews, hiring, and mentoring.
- Works closely with community healthcare partners to promote and provide education about RHSP.
Quality Assurance
- In conjunction with the Executive Director, supports the continuous quality improvement program in
place.
- Ensures appropriate, prompt, and equitable admission and/or discharge decisions.
- Investigates and is responsive to any concerns regarding resident care and reports findings to
Executive Director
- Ensures operational efficiency and is responsible for applicable budget lines.
- Collects relevant data that provides for measurement of outcomes with respect to defined quality
indicators and metrics and monitors trends.
- Ensures compliance with all applicable legislation and HPCO Accreditation standards.
Qualifications & Experience
- Current Certificate of Competence issued by the College of Nurses of Ontario
- Certification in CAPCE and completion of LEAP a benefit
- Current Canadian Hospice Palliative Care Nurse Certificate or willingness to pursue and complete
within two years of employment
- In-depth understanding and clinical experience in palliative care at the end-of-life stage of the
person's illness trajectory, with a preference for hospice experience
- Proven consultative team-based management experience including leadership and administration in
a palliative approach to care.
- Positive approach to resident/client service and a demonstrated capacity for empathy
- Experience in working with a wide variety of community service providers.
- Proven ability to work independently, skilled at critical thinking to make appropriate decisions, and
to accept responsibility for outcomes.
