About the role

Position Description –Counselling Program Facilitator

The incumbent for this position will have at a minimum a Bachelor of Social Work Degree from an accredited university and/or an equivalent combination of experience plus a diploma from a community college in a related field.

The incumbent will bring a combination of direct experience in providing case management and programming to at risk youth and adults along with an understanding of and ability to relate to the diversity of demographics within Toronto. The incumbent will have excellent conflict resolution skills, case management and group work skills and an understanding of gang life and gang culture within Toronto and use of trauma informed approach.

The incumbent will be familiar with Cognitive Behavioural Interventions and identification of criminogenic factors, using Motivational Interviewing and Stages of Change to assist individual intervention plans. You will have demonstrable experience working independently while maintaining a high degree of accountability and can demonstrate success in community engagement and collaboration while continuing to improve and develop programming.

The role is to assist clients in following through on plans, to serve in a brokerage role in linking client with assistance, and to provide crisis intervention services in assisting clients to maintain gains towards their personal development. The incumbent will liaise with a variety of community resources, referral sources, and specialized services in facilitating achievement of client goals.

The focus of work for this position will include responsibility for the development of individual participant goals and plans, facilitation of workshops geared to personal development and opportunities for positive change for program participants. You will also be expected to coach/mentor and provide participants with relevant and evidence-informed service interventions to promote assets, competency, and resiliency. You will assume service coordination functions while acting as an advocate and providing information and referrals to program participants. Other duties include mentoring and counselling program participants to encourage social inclusion and engaging these program participants to identify issues, needs, goals, aspirations, skills, abilities, and services needed as well as barriers to accessing these services.

The incumbent for this position will be responsible for the following

Intake and Assessment including Individual Planning and Case Management Maintaining case recording on client interactions as appropriate and as per agency guidelines, leading to overall assessment of client performance and impact of service Facilitation of Personal Development Workshops and Individual Sessions including de-briefing and compliance with internal/external reporting requirements Assigned Other Duties in Accomplishing Goals of the Program Ensuring accuracy, confidentiality and safekeeping of agency records Participating constructively in the supervision process with the Manager of Programs and exercising due diligence with respect to self-care and accountability for practice

The Counselling Program Facilitator will work collaboratively with Team members and will be under the direct supervision of the Manager of Programs. Although most of the work will take place between 9 a.m. – 5 p.m. Monday to Friday, some evening work may be required. A valid Ontario driver’s license with access to a reliable vehicle is considered an asset.

Salary range for this position is $50,000 to $55,000 plus benefits when eligible.
