Job Description:

Position Overview:
Reporting to the Patient Care Manager, you will work independently within your scope of practice when the patient’s condition indicates predictable outcome and you will deliver nursing care based on a pre-set plan of care with a delineated range of intervention. You will collaborate with and get directions from the RN in situations that are beyond the scope of complexity designated to you as a RPN. You will be part of a busy department that provides excellent care to a diverse patient population within our multi-service environment who have been designated Alternate Level of Care (ALC) and require short stay intensive transitional care for discharge. The model of care on this unit is a team based model working closely with Personal Support Workers (PSW) and Registered Nurses (RN) along with allied health team. You will provide high quality nursing care to patients and their families, based on the philosophy and conceptual framework of Nursing Practice at Scarborough Health Network.

Requirements:

  • Current registration with the College of Nurses of Ontario

  • Current BCLS

  • Recent relevant experience in geriatric / rehabilitation or acute medicine

  • Completed medication course for RPN's

  • Patient assessment skills within the RPN scope of practice

  • Willingness to work with mixed aggregation of patients

  • Excellent interpersonal and communication skills both written and verbal

  • Demonstrates excellent problem-solving, decision making and critical thinking skills

  • Demonstrates excellent organizational, time-management and team building skills

  • Demonstrates initiative and the ability to function both independently and as a multidisciplinary team member in a fast paced demanding environment

  • Demonstrates values consistent with SHN’s Code of Conduct and is committed to fostering a healthy and positive work environment in accordance with Health & Safety policy and legislation.

  • Utilize protective equipment provided and identify and reports hazards

  • Engages in ethical conduct and strives to understand and appreciate the diversity of our patient/staff population and community

  • Strong work ethic and commitment is required

  • Demonstrated high standards of work outputs

  • Highly punctual

  • Demonstrated professionalism and reliability

Asset:

  • Certification in Gerontological Nursing
  • Previous work on medicine and/or Transitional Care Unit
  • Graduate of an RPN diploma program
  • Patient Assessment course
  • Ability to speak another language
