Position Profile:
The Community Integration Worker facilitates youth in increasing accessibility, improving community integration and social inclusion by connecting with community resources such as life skills, recreational, educational, occupational, and vocational activities. The Community Integration Worker will advocate for youth to access appropriate services and support within the community and through systems navigation, guidance, and referrals.
Role Overview
Responsibilities:
- Facilitate youth in improving community integration and social inclusion by connecting with community resources as well as recreational, educational, occupational and vocational activities
- Offer supplementary case management support, including the development of integration plans and community mapping, to assist youth in successfully transitioning into stable housing.
- Advocate for youth to access appropriate services and support within Our Place Peel and the community and through systems navigation guidance and referrals.
- Assist youth in developing and following through with comprehensive case plans designed to concentrate on obstacles preventing success in housing as well as promoting longer term stability and achievement.
- Coordinate and/or facilitate group based and one-on-one life skills programs onsite and in the community.
- Provide a monthly calendar in advance for the youth, consisting of internal and external programing.
- Facilitate monthly resident meetings and administer resident satisfaction surveys
- Actively engage all youth of the residence on a daily basis to participate in programming or encourage development of life skills.
- Actively network within the community to promote the program and services by conducting presentations, attending information sessions and sitting on committees.
- Build and maintain relationships with a broad spectrum of partner agencies and community service agencies to increase referral and intake sources, including culturally appropriate partnerships
- Create opportunities for youth to develop positive social relationships particularly for subpopulations including indigenous, 2SLGBTQ+, newcomer youth etc.
- Plan and deliver culturally appropriate programming with the overall goal of promoting social including, building strength and enhancing skills.
- Compile statistical information and prepare monthly reports as required for program evaluation and development.
- Participate on committees and task forces as required with other agencies addressing issues facing youth.
- Create a safe and secure setting for all youth accessing services at the transitional program.
- Ensure that professional standards are maintained as related to client documentation and case management.
- Ensure that the facility meets health and safety standards.
- Foster a positive and supportive working environment.
- All other duties as required in the successful management of shelter operations while partnering with all stakeholders in support of organizational success.
Qualifications:
- Diploma in Child and Youth Worker or equivalent in related social services field.
- Experience within the social service sector supporting youth experiencing homelessness in obtaining housing, employment, educational and community opportunities.
- Expertise in leading and developing programming for young people with comprehensive needs.
- Cultural competency sensitivity and commitment to working through an EDIB, anti-oppressive, anti-racism framework.
- Proficiency in data collection and statistical reporting an asset.
- Knowledge of trauma informed care, and harm reduction practices an asset.
- Understanding of issues relating to youth homelessness and the barriers youth face in attaining and maintaining their housing, as well as an understanding of the broader housing sector in Peel.
- Valid CPI/UMAB and First Aid/CPR certifications.
- WHMIS and Food Handling Certification recommended.
- Completion of a successful Vulnerable Sector Check/Criminal Record Check will be a condition of employment.
- Fully vaccinated against COVID-19, subject to applicable legislation.
