Our Social Prescribing Program located in Innisfail is seeking a: Full-Time Link Worker

The Social Prescribing for Older Adults Program is funded through Healthy Aging Alberta and has been created to bridge the gap between medical and social care services through a holistic approach. Through this approach, healthcare professionals refer patients to community-based programs to improve their health and enhance their quality of life. It is a means for trusted individuals in clinical and community settings to identify that a person has non-medical, health-related social needs and to subsequently connect them to non-clinical supports and services within the community by co-producing a social prescription – a non-medical prescription, to improve health and well-being and to strengthen community connections.

Minimum Qualifications and Requirements:

  • Required high school diploma, preferred post secondary certificate or diploma from an approved institution related to human services or health care.
  • Required 3 years of community work with vulnerable populations, preferred 3 years of frontline work with aging adults
  • Knowledge and experience in the health care field
  • Experience in community connection and system navigation
  • Knowledge of the healthy aging framework
  • Basic First Aid Level C with Adult, Child, and infant CPR &AED
  • Suicide Intervention Training & De-Escalation Training
  • Intervention Record Check & Criminal Record Check with the vulnerable sector
  • Class 5 driver’s license, clean drivers abstract and reliable vehicle for client transportation.

Responsibilities include:

  • Assist older adults to find and access community support through referrals and providing information.
  • Assist older adults with completion of social benefit applications (AISH, Housing, AB Seniors Benefits, etc.)
  • Attend appointments with older adults when appropriate to help overcome barriers.
  • Collaborate with other community agencies.
  • Develop personalized goals with older adults and help to achieve them.
  • Act as a compliment to the older adult’s current medical care team.
  • Maintain and report on program statistics and outcomes.

Wage: $23.46/hr. - $26.24/hr. Plus, Travel Reimbursement at $0.57/KM.

Hours of work: 40 hours per week. Candidates must be available to work a flexible schedule including afternoons, some evenings and weekends to provide services that serve a wide range of populations.
