Job Description:

As a member of the Clinical Resource Team (CRT) you will have the opportunity to work across programs and across Trillium Health Partners sites. The CRT staff is a vital component of ensuring uninterrupted patient care delivery by providing coverage for planned absences of inpatient Occupational Therapists. This position requires an individual who is highly skilled, agile and adaptable with excellent time management and interprofessional skills. Within the CRT Operations and Practice are supported by a Manager, Educator and the Professional Practice Leader for Occupational Therapy. Key Qualifications:

  • Degree in Human Nutrition and Dietetics combined with an accredited Dietetic Internship program
  • Current registration with the College of Dietitians of Ontario
  • Membership in Dietitians of Canada
  • Demonstrated leadership and communication skills
  • Positive work and attendance record essential
  • Reliable, flexible, creative, self-directed professional, able to work effectively and collaboratively within a team environment
  • ICU experience an asset Flexibility to cover multiple areas

Position Summary:

  • As an active member of the multidisciplinary team, you will be responsible for the delivery of client-centered nutritional counseling to patients in acute, rehab and complex care
  • Provide assessment, contribute to care plan development, establish goal-oriented treatment interventions, and provide patient/family education
  • Demonstrated commitment to ongoing professional development and student learning
  • Acts as a resource for other health care providers and the community as required
  • Experience supervising dietetic interns an asset Experienced in the management of enteral and parenteral nutrition
