Position Summary:

  • As an active member of an interdisciplinary stroke team the candidate will be responsible for the delivery of client-centered nursing services for assigned patients.
  • In collaboration with an RN provides nursing assessment, care plan development, goal oriented treatment interventions, patient/family education and support to address medical cognitive, emotional and self-care needs within scope of practice.
  • Contributes to the development of a realistic discharge plan.
  • The ability to perform as a coach/mentor/consultant and to use sound critical thinking skills is key to achieving patient outcomes.
  • Participates in program planning, continuous quality improvement and evaluation.

Key Qualifications:

  • A current annual registration card with the College of Nurses of Ontario.
  • Recent (2 years) Medication and Aseptic Technique Certifications.
  • Current Physical Assessment course preferred.
  • Recent (1-3 years) experience with advanced clinical skills in management of Stroke patients throughout the continuum of care from acute through rehabilitation.
  • Neurological/neurosurgical, cardiac and medical experience would be an asset.
  • Current BCLS and evidence of continued education required.
  • Combined with sound patient care nursing skills must have effective interpersonal and organizational skills.
  • Reliable, flexible, creative, self-directed professional able to work effectively and collaboratively with the Stroke Team.
  • Recent (1-3 years) experience working with a high performance interdisciplinary team required.
  • Demonstrated effective interpersonal, organizational and communication skills are required.
  • Positive attendance and work record essential.
  • Must meet the physical demands of the position; performing substantial walking, standing, lifting, twisting and bending as required for patient care and other responsibilities.
  • Stroke experience an asset.
  • Positive work and attendance record is essential.
