Charles Taylor is a highly successful global provider of professional services to the insurance industry. We are seeking an experienced Field Case Manager to join our team. This is a remote position traveling in Southeastern KY.
Position Summary
This position has the responsibility to identify, assess, plan, coordinate, implement, monitor and evaluate options and services for individuals requiring medical and/or vocational case management services. The goal of the Case Manager is to provide quality, cost effective outcomes for the client through collaborating and communicating with the claimant, the family, the physician, and other members of the health care team.
Essential Functions
- On-site visits with the claimant, medical care provider and employer, as needed, to evaluate treatment plans, assess the injured worker, family and providers. May require lengthy travel to appointments and requires ability to walk distances to and from medical facilities.
- When needed, visits injured worker home/ family to assess and identify strengths, resources, problems, psychosocial, financial and medical history. Evaluates current status, diagnosis, prognosis, functional status, current treatment plan and needs.
- Develops a plan for physical/ medical and/or vocational services through interdisciplinary collaboration, which identifies realistic short and long-term goals.
- Coordinates the services and resources to implement the plan. Negotiates or utilizes approved vendors for the highest quality, most cost-effective appropriate services.
- Demonstrates effective, accurate and timely verbal and written communication. Requires the employee to sit for periods, utilize computer software and telephone to complete required case management activity and prepare reports to clients.
- Provides vocational plan for rehabilitation designed to facilitate injured worker’s return to suitable, gainful employment. Includes visits to work sites when requested or required by the client/ employer.
- Meets Quality Management standards of Charles Taylor.
- Participates in marketing activities as requested by Manager or Assistant for a prescribed geographical area. Must be able to drive long distances when required.
Qualifications and Educational/Licensing Requirements
- Degree in nursing or other degree and appropriate experience as follows:
- RN with minimum of 3 years direct patient care.
- Current RN licensure, if applicable.
- Case management/ Vocational Rehab experience preferred.
- Current CCM or other applicable certification or eligible for certification.
- Strong clinical knowledge base.
- Excellent communication skills; written and verbal.
- Strong analytical skills.
- Ability to work independently under general management direction to complete all assignments.
- Ability to prioritize and organize tasks.
