Located approximately 100 kilometres east of Toronto, NHH delivers a broad range of acute, post-acute, outpatient and diagnostic services. Acute services include emergency and intensive care, medical/surgical care, obstetrical care and palliative care. Post-acute specialty services (PASS) include restorative care and rehabilitation. Mental health care, cancer and supportive care, dialysis and other ambulatory care clinics are offered on an outpatient basis through partnerships with regional centres and nearby specialists. NHH offers a full range of diagnostic services, including magnetic resonance imaging (MRI), computed tomography (CT) and mammography. The hospital serves the catchment area of west Northumberland County. A mixed urban and rural population of approximately 67,000 residents, west Northumberland comprises the Town of Cobourg, the Municipality of Port Hope, Alderville First Nation and the townships of Hamilton, Cramahe and Alnwick/Haldimand. ROLE OVERVIEW
Reporting to the Clinical Manager, Access and Flow, the Social Worker provides services to patients and their families. The inpatient Social Worker will focus on the psychological and emotional well-being of individuals as they adjust to changes in their health status. This role will also support intensive case management as patients and families navigate the health care system.
QUALIFICATIONS/REQUIREMENTS

Formulate comprehensive psychosocial assessments of complex case situations or specialized problems
-
Design and implement programs for complex case situations or specialized problems.
-
Demonstrate expertise in all areas of clinical practice (i.e., individuals, families, and groups).
-
Demonstrate expertise in a broad range of knowledge i.e., bio-psycho-social dynamic of illness and disease
-
Demonstrate knowledge, understanding and leadership skills in health-related issues.
-
Demonstrate experience in crisis intervention skills and critical incident debriefing
-
Demonstrate skill in the use of systems theory in a hospital environment.

Participate as a member of the interdisciplinary team and consults with interdisciplinary team members
-
Participates in patient flow activities including discharge planning
-
In addition to the above, follows safe and healthy work practices and maintains a safe and healthy work environment through attendance at educational programs, use of established reporting mechanisms for injuries, illness and hazards and adherence to health and safety related policies and procedures

MSW or evidence of participation in a MSW program
-
Member of the Ontario College of Social Workers and Social Services Workers (OCSWSSW) in good standing
-
3 to 5 years of previous experience in a similar role in a hospital

Demonstrated knowledge of local community resources including LTC agencies
-
Knowledge of various government legislation and support programs (Substitute Decisions Act, Spousal Death Benefits etc.)
-
Experience working with LTC and cancer care clients
-
Ability to function independently and as part of a team
-
Excellent written, verbal, and interpersonal
EMPLOYMENT REQUIREMENTS
Successful applicants are required provide a satisfactory vulnerable sector criminal reference check, that was complete less than one year from the date of hire. This requirement must be met within the first sixty (60) days of employment.
Additionally, to the above, successful applicants are also required to complete the Health Care Worker Health review process which includes providing an up-to-date immunization record to ensure compliance with the Ontario Hospital Association Communicable Diseases Surveillance Protocols within the first thirty (30) days of employment. Northumberland Hills Hospital operates a mandatory COVID-19 vaccination policy for all staff.
