Position Summary
The Primary Care Dietitian works within a team-based environment in Mosaic PCN clinics and physician offices. The primary purpose of the role is to provide nutritional expertise to address the needs of patients experiencing nutrition-related conditions/diseases. The Primary Care Dietitian strives to improve health through nutrition strategies consistent with population health promotion. The focus is on achieving outcomes to meet the Mosaic PCN objectives.
The Primary Care Dietitian assesses, plans, implements, and evaluates appropriate and optimal nutritional care with the goal to improve nutritional and health status. The role consults with patients both one on one and in group settings.
Key Responsibilities
Assessment
- Assesses the nutritional status of individuals, families, and communities through screening, data collection, ongoing monitoring, and reassessment.
Intervention
- Educates individuals, families, and communities to assist in applying nutrition principles to improve overall health status using motivational interviewing and behavioural change management.
- Provides clinical services using a patient centred approach in both one to one and group settings.
- Identifies priorities, target dates, and strategies, together with the patients, which contribute to individualized treatment plans and goals.
- Evaluates the extent to which outcomes have been achieved and recommends alternative treatments as necessary.
- Consults directly with family physicians and provides referring physicians with concise verbal or written feedback in a timely manner.
- Develops and delivers nutrition education and health promotion programs targeting populations of concern.
Inter-professional Collaboration
- Works collaboratively with the multidisciplinary team to assess, plan, implement, and evaluate patients’ needs to ensure they receive appropriate and optimal care.
- Mentors colleagues and dietetic interns in areas of expertise and seeks mentorship to achieve full potential in professional development.
Documentation
- Documents all phases of patient care in the appropriate Electronic Medical Record (EMR) to maintain accurate and timely records, as well as recording the details of clinical interactions in MPCN evaluation systems.
Community Engagement
- Participates in community activities within the Mosaic PCN catchment area, including health fairs, conferences, and other community events to promote Mosaic programs and services.
Professional Development
- Identifies and works to achieve own professional development needs which support goals set in performance reviews.
Population Health Programming
- Participates in and informs the ongoing operation, planning, and evaluation of community-based population health programs.
Other responsibilities as required.
Qualifications & Requirements
Education & Licenses
- Bachelors’ degree in nutrition.
- Licensed within the College of Dietitians of Alberta (CDA).
Experience
- A minimum of 3 years of relevant experience, ideally in public health, health promotion or community-based nutrition.
- Experience working with ethnically diverse, vulnerable, and high risk populations.
Knowledge, Skills & Abilities
- Demonstrated ability in planning, developing, implementing, and evaluating nutrition strategies that is consistent with population health promotion.
- Knowledge of nutrition recommendations and concepts.
- Effective communication skills both verbally and written in English.
- Computer knowledge literacy including MS Office and Outlook/Calendar management.
- Sound judgment and ability to develop collaborative working relationships.
- Self-directed, flexible, and displays initiative.
- Strong time management, prioritization, and organizational skills.
Assets
- Certified Diabetes Educator (CDE) certificate.
- International Board-Certified Lactation Consultant (IBCLC) certificate.
- Pediatric counseling experience.
