• **Job Summary ** We are seeking a compassionate and dedicated Registered Practical Nurse (RPN) to join our healthcare team. The ideal candidate will provide high-quality patient care across various settings, including nursing homes, hospitals, assisted living facilities, and home care environments. This role requires a strong understanding of medical procedures, patient needs, and effective communication to ensure optimal health outcomes for diverse patient populations, including seniors, children, and those requiring hospice or dementia care. **Duties **
• Deliver comprehensive patient care in accordance with physician orders and established protocols.
• Administer medications and treatments accurately, including injections and phlebotomy procedures.
• Monitor patient vital signs and assess health status regularly.
• Assist with pain management, wound care, and other specialized treatments such as dementia or hospice care.
• Utilize electronic health records systems such as Epic and Cerner for documentation and case management.
• Collaborate with multidisciplinary teams to develop and implement individualized care plans.
• Educate patients and their families on health conditions, medication use, and post-discharge care instructions.
• Maintain accurate medical documentation adhering to confidentiality standards and legal requirements.
• Participate in quality improvement initiatives and staff training sessions to enhance patient outcomes.
• Support laboratory procedures when necessary, including specimen collection and processing. **Qualifications **
• Valid registration as a Registered Practical Nurse (RPN) with the appropriate licensing body.
• Prior experience in nursing home, hospital, or acute care settings preferred; experience with dementia, pediatrics, senior care, hospice, or home care is a plus.
• Proficiency in medical terminology, medication administration, and patient assessment skills.
• Experience with electronic health records systems such as Epic and Cerner is highly desirable.
• Strong communication skills with the ability to work collaboratively within a healthcare team.
• Knowledge of laboratory procedures including phlebotomy is advantageous.
• Ability to handle multiple priorities efficiently while maintaining attention to detail.

Requirements

• Current Certificate of Competence and in good standing with the provincial College of Nurses
• Graduation from a recognized program for Practical Nurses and one year of recent related experience in gerontological nursing.
• Functions within the limits of own experience and knowledge, and practices safely and competently within the job description.
• 1 year of nursing experience required; three years nursing experience preferred.
• Criminal Record Check with a Vulnerable Sector Search
• Negative TB test or chest x-ray
• Assists clients in all activities of daily living/hygiene assistance, bathing, grooming, linen changes and so forth
• Community, home health or medical/surgical experience preferred.
