- **Job Summary ** We are seeking a compassionate and dedicated Registered Practical Nurse (RPN) to join our healthcare team. - Deliver comprehensive patient care in accordance with physician orders and established protocols.
- Administer medications and treatments accurately, including injections and phlebotomy procedures.
- Monitor patient vital signs and assess health status regularly.
- Assist with pain management, wound care, and other specialized treatments such as dementia or hospice care.
- Utilize electronic health records systems such as Epic and Cerner for documentation and case management.
- Collaborate with multidisciplinary teams to develop and implement individualized care plans.
- Educate patients and their families on health conditions, medication use, and post-discharge care instructions.
- Maintain accurate medical documentation adhering to confidentiality standards and legal requirements.
- Participate in quality improvement initiatives and staff training sessions to enhance patient outcomes.
- Support laboratory procedures when necessary, including specimen collection and processing. **Qualifications **
- Valid registration as a Registered Practical Nurse (RPN) with the appropriate licensing body.
- Prior experience in nursing home, hospital, or acute care settings preferred; experience with dementia, pediatrics, senior care, hospice, or home care is a plus.
- Proficiency in medical terminology, medication administration, and patient assessment skills.
- Experience with electronic health records systems such as Epic and Cerner is highly desirable.
- Strong communication skills with the ability to work collaboratively within a healthcare team.
- Knowledge of laboratory procedures including phlebotomy is advantageous.
- Ability to handle multiple priorities efficiently while maintaining attention to detail.

Requirements
- Current Certificate of Competence and in good standing with the provincial College of Nurses
- Graduation from a recognized program for Practical Nurses and one year of recent related experience in gerontological nursing.
- Functions within the limits of own experience and knowledge, and practices safely and competently within the job description.
- 1 year of nursing experience required; three years nursing experience preferred.
- Criminal Record Check with a Vulnerable Sector Search
- Negative TB test or chest x-ray
- Assists clients in all activities of daily living/hygiene assistance, bathing, grooming, linen changes and so forth
- Community, home health or medical/surgical experience preferred.
