Resident and Family Services Coordinator

Meadow Park London is a fully accredited long term care home in London. Ontario and serves 126 residents. It is a growing urban center in Southwestern Ontario, and is known for its numerous parks, cultural attractions, and strong economy in sectors like agri-food, advanced manufacturing, and health sciences. Situated on the Thames River, London is located approximately 200 km west of Toronto and 200 km east of Windsor, it serves as a regional hub for medical, educational, and business services.
Join our team as a Resident and Family Services Coordinator!
Under the direction of the Executive Director, the Resident and Family Services Coordinator is responsible and accountable for providing a smooth transition for resident admission and continued placement. Acting as a resident advocate, this role ensures and provides support to residents and families before, during and following the Admission Process.
Key responsibilities also include maintaining strict confidentiality, facilitating effective communication among residents, families and staff, acting as a liaison between Ontario Health atHome and Discharge Planners and participating in Continuous Quality Improvement Activities measuring outcomes through Admission Surveys and Annual Resident/Family surveys.
We are looking for the following qualifications and traits**:**
- Must have a current general certificate of registration with a college of a regulated health profession or the Ontario College of Social Workers and Social Service Workers; or
- Must have a post-secondary diploma or degree in recreation and leisure studies, kinesiology, therapeutic recreation or other related field from a community college or university, and at least one year of experience in a health care setting as required under the Fixing Long Term Care Act, 2021, ss 79 and Ontario Regulation ss. 70/22 (1).
- Minimum of 2 years experience Long Term Care (Gerontology) preferred
- Must demonstrate an active interest in ongoing education
- Must demonstrate good communication skills
- Must have an understanding of the Nursing Processes
