Position Summary
Reporting to the Director, Emergency, Mental Health and Patient Flow, the Manager of Patient Flow provides leadership and operational oversight to ensure patients receive exceptional, timely, and safe care. This role is accountable for clinical excellence, strategic planning, and team leadership across the patient flow processes.
Working collaboratively with internal teams and external partners, you will lead initiatives that enhance patient flow, reduce ED to Inpatient Bed metrics aligning with OH goals, reduce ALC days, support Home First strategies, and advance organizational goals.
Key Responsibilities
Patient Flow Leadership:
- Develop and implement strategies to improve patient flow across all units, ensuring timely access to care.
- Oversee the work of Discharge Coordinators, to support safe and efficient transitions, aligned with Home First principles.
- Lead initiatives to reduce ALC days and improve discharge planning.
- Collaborate with the Program Director and ED Manager to operationalize strategies for the ED Pay for Results Program
- Collaborate with IPAC leadership around IPAC best practices while maximizing bed utilization
- Monitor and report on flow metrics.
- Analyze patient flow data and performance metrics to inform decision-making, set priorities, and drive sustained improvements in access, throughput, and discharge outcomes.
- Collaborate with clinical teams to resolve barriers to discharge and optimize bed utilization.
- Lead, sponsor, and evaluate quality improvement initiatives using structured methodologies (e.g., LEAN, PDSA), ensuring alignment with organizational and Ontario Health priorities.
- Lead strategies for change management and process improvements in patient flow across the organization while engaging multiple stakeholders
- Ensure team adherence to regulatory, professional, accreditation, and patient safety standards, embedding these requirements into operational practice and performance expectations.
- Provide coaching, mentorship, and performance feedback to team members to support professional growth, role clarity, and leadership development.
- Lead teams through complex, high-pressure system challenges, balancing operational demands with staff wellbeing and patient safety.
Department/Unit/Program Description
At Ross Memorial Hospital, we are committed to delivering exceptional care and optimizing patient flow across the organization. The Patient Flow team plays a critical role in ensuring timely admissions, transfers, and discharges to support safe and efficient care delivery. This includes oversight of the discharge planning team. This team leads strategies to reduce Alternate Level of Care (ALC) days and promote the Home First philosophy, ensuring patients transition to the most appropriate setting as soon as possible.
Essential Qualifications
- Active registration in good standing with a relevant professional college (e.g., CNO)
- Baccalaureate degree in a health discipline
- Minimum three (3) years of recent management experience in a healthcare setting
- Minimum five (5) years of hospital experience with demonstrated clinical knowledge
- Strong interpersonal, communication, coaching, conflict resolution, and negotiation skills, with the ability to build trust and foster respectful collaboration.
- Demonstrated ability to lead change, exercise sound judgment, and provide effective leadership in complex, high pressure healthcare environments.
- Strong analytical, organizational, and decision making skills supported by systems thinking and continuous improvement principles.
- Demonstrated commitment to patient and family centred care, professional development, and evidence informed practice.
- Demonstrated understanding of social determinants of health and their impact on discharge planning, length of stay, patient flow, and system capacity.
- Must be able to fulfill physical requirements of the job, as per current Physical Demands Analysis.
Preferred Qualifications
- Master's Degree in Health Administration or other relevant Post Graduate education considered an asset
- Lean Certification considered an asset
Hours of Work
Hours of work for the Manager position are 37.5 hours/week, Monday to Friday. You will also be scheduled on a Manager on Call rotation that includes nights and weekends.
