###Position Summary
Foundation Health is seeking a skilled and collaborativeEndocrinologistto join our expanding team. The successful candidate will provide consultative and ongoing care for patients with a wide range of endocrine and metabolic disorders, working closely with primary care providers and allied health professionals.
This role offers flexibility in scheduling, referral volumes, and the opportunity to build a focused or general endocrinology practice.
###Key Responsibilities
- Provide assessment, diagnosis, and management of endocrine disorders, including:
- Diabetes (Type 1, Type 2, gestational)
- Thyroid disorders
- Adrenal and pituitary conditions
- Osteoporosis and metabolic bone disease
- Lipid disorders and metabolic syndrome
- Interpret and manage diagnostic tests (laboratory, imaging, dynamic testing)
- Collaborate with family physicians and other specialists to ensure continuity of care
- Provide patient education and support for chronic disease management
- Maintain accurate and timely clinical documentation in the EMR
- Participate in quality improvement initiatives and interdisciplinary case discussions
###Qualifications
- Medical Degree (MD) with certification in Internal Medicine and Endocrinology
- Fellowship with theRoyal College of Physicians and Surgeons of Canada (FRCPC)
- Licensed or eligible for licensure with theCollege of Physicians and Surgeons of Ontario (CPSO)
- Strong clinical and communication skills
- Commitment to collaborative, team-based care

Foundation Health operates in many healthcare verticals, including medical supply retail/wholesale, staffing services, virtual mental health, and private referral services for home care and Allied Health. In addition, we procure and operate primary care clinics in established and greenfield settings.
Our primary care clinics are staffed with CPSO-certified family doctors, administrative staff, and nursing and care coordinators, offering multidisciplinary care in a team-based setting. Many of our clinics are located in or attached to pharmacies to round out our comprehensive care model with our pharmacist partners.
