Working within a multi-disciplinary team framework the Transitional Case Manager performs duties and responsibilities consistent with providing a psychosocial environment to assist with the rehabilitation and community integration of individuals involved in the criminal justice system, including those with serious and persistent mental health concerns, developmental disorders, acquired brain injury and/or concurrent disorders.

WHAT WE'RE OFFERING:

What you'll be making - Starting rate for this role is ($30.01 per hour for Level 1 – Direct Service, Regulated):
- Successful candidates must be registered with one of the designated professional colleges (e.g. OCSWSSW, CNO, COTO, CRPO, CPSO, or CPO) will receive the wage differential for regulation
- Successful external applicants will start at Level 1, with grid progression based on seniority up to Level 8 ($37.26 per hour for regulated)
- For internal applicants, grid level placement will occur as per the Collective Agreement

When you'll work – 70 hours over a biweekly period; Monday to Friday 8:30 am to 4:30 pm.

HOW YOU WILL BE MAKING A DIFFERENCE:
Assessment -In collaboration with the individual:

Complete intake/screener to support referral to Adult Therapeutic Court
-
Gather health related documentation as required for Adult Therapeutic Court
-
Conduct assessments, treatment planning, and reporting for the Mental Health Diversion Program
-
Provide information and referral support to individuals, family members, and service providers
-
High level assessment skills to assess clients’ mental, physical and emotional functional abilities and needs.
Individualized Planning - utilizing the information obtained through the assessment process and in collaboration with the individual:
- Assist the individual to identify personal goals and develop support plans that identify individual strengths and areas for personal growth and goals to meet determined needs.
- Provide 1:1 support services to a caseload of individuals with mental health concerns who are currently involved in the criminal justice system
- Provide skills teaching and support to clients to assist in goal achievement
- Provide assistance with financial management, vocational and occupational support, and discharge planning.
- Provide crisis support and crisis prevention planning
- Engage in group facilitation as required that supports clients in completing their established goals for court diversion
Service Coordination and Advocacy:
- Have knowledge of specialized courts
- Have knowledge of community resources relevant to individuals, including other mental health and addiction services, entitlement and benefit programs, (e.g. ODSP, Ontario Works, etc.)
- Attend court sessions, as needed, and provide court support to individuals dealing with current or past charges
- Liaise with legal professionals and service providers to ensure a coordinated approach to service provision
- Advocate for client rights as necessary and support self-advocacy
- Identify, develop and maintain good links with a wide range of community resources
- Work collaboratively with other service providers to ensure a well-coordinated approach to provide support and/or discharge planning from detention centres/forensic hospital, etc.
Documentation:
- Maintain current and accurate client clinical documentation records as per agency policy
- Strong skills and ability to provide written and verbal reports to justice partners including judge, crowns, defence attorney, probation and parole
Teamwork:
- Attend and actively participate in staff and team meetings
- Ongoing professional skill development/continuing education toward the delivery of mental health services
- Protecting and promoting the welfare, dignity, and individual rights of clients at all times, understanding trauma and violence and the impact these have on clients’ lives and behaviours
- Triaging and providing initial mental health and/or addiction screening, coordinate and facilitate with client linkages to recommended services and supports in the community
- Providing health teaching and information regarding a variety of coping techniques and strategies, e.g. grounding, breathing techniques, self-care, nutrition, sleep hygiene, techniques to enhance overall well-being, the completion of crisis plans
- Supporting clients to secure housing of their choice; and to successfully remain in their housing, providing prevention and diversion strategies to avoid homelessness
- Providing outreach to the homeless population, ensuring connection to services and that basic needs are met
WHAT YOU'LL NEED TO APPLY:
Police Vulnerable Sector Check Requirements
Successful candidates must have a Police Vulnerable Sector Check completed in the last six months. Interested applicants who reside in the City of London and are in need of a Police Vulnerable Sector Check (PVSC) are encouraged to apply for one as soon as possible as PVSCs in the City of London are currently taking approximately 2 weeks to be processed and offers of employment cannot be made without receipt of a PVSC.
We require the following qualifications:
- Registration with a relevant professional college is required (currently recognized colleges include OCSWSSW, CNO, CRPO, CPSO, COTO, CPO)
- Post secondary degree or diploma in related field
- Valid Driver’s Licence, adequate insurance and access to a vehicle to carry out job duties
- Demonstrated ability to attend work regularly
We prefer the following qualifications:
- Minimum of 3-5 years’ experience working in mental health, preferred
- Current CPR and First Aid Certification
- ASIST Certificate
- Experience working within the criminal justice system
- Experience and detailed knowledge of serious mental health concerns, acquired brain injury, developmental disabilities
- Experience working in a community-based setting
- Working knowledge of a variety of treatment, interventions, and support strategies
- Experience in crisis intervention/management and crisis resolution
- Extensive knowledge of services in the Thames Valley region
- Knowledge of the legal system
- Planning and organizational abilities
- Excellent communication skills
- Proven ability to work in collaboration with others
- Strong active listening and verbal communication skills
- Computer skills
