Key Responsibilities

Document, review, verify, check, and dispense all prescribed medication orders for appropriateness. Identify, resolve, and prevent drug-related problems. Monitor drug therapy for effectiveness, toxicity, and adherence to evidence-based medicine and clinical guidelines. Apply knowledge of drug theory, evidence-based pharmaceutical practice, current treatment modalities, and community pharmacy resources. Maintain knowledge of Ontario College of Pharmacists professional standards and guidelines, as well as legislation related to the practice of pharmacy, including the Controlled Drugs and Substances Act, Federal Food and Drug Act, Mental Health Act, and other applicable legislation. Conduct medication reconciliation. Record and report clinical activities in accordance with organizational policies and Regulatory College requirements. Follow up on medication incidents and promote safe medication practices. Respond to drug information inquiries from patients and healthcare professionals. Provide direct and/or functional supervision of pharmacy technician staff. Perform other duties consistent with the scope of practice of a registered Pharmacist. Handle blister packs and medication packaging. Demonstrate familiarity with common drug regimens and dispensing processes for specialty medications. Maintain working knowledge of adherence programs, including but not limited to compliance packaging, refill synchronization, and refill management. Adapt communication skills to provide medication information, health education, and counselling to diverse patient populations. Support patients in the implementation and modification of their treatment plans. Evaluate patient responses to treatment and modify treatment plans to achieve established therapeutic goals. Educate patients regarding health self-care, medication use, and preventive care. Diagnose and treat acute medical conditions and/or injuries within the pharmacist's authorized scope of practice. In consultation with the Client's healthcare team, provide referrals to community specialists when appropriate. Review consultation notes from community healthcare providers and integrate recommendations into patient care as appropriate. Dispense medications in accordance with the Client's approved Drug Formulary. Maintain pharmacy standards, including inventory management of medications, medical supplies, and equipment within allocated resources. Support and utilize health technology initiatives implemented by the Client, including electronic medical records (EMR) and provincial electronic health records where available. Collaborate with the Client's healthcare manager to provide advice and support strategic planning for the delivery of healthcare services. Communicate in a timely manner with the Client's healthcare professionals regarding patient care. Attend required meetings and teleconferences related to the provision of healthcare services, including policy implementation and operational updates. Arrange for qualified replacement pharmacist coverage during absences. Maintain and monitor an email account for communication with Supply Ontario and the Client. Maintain the capability to receive and transmit documents and prescriptions by fax. Comply with and adhere to all security requirements of the Client's facility or facilities.

Qualifications and Experience

Be in good standing and hold a valid licence issued by the Ontario College of Pharmacists (OCP) to practice as a Pharmacist within the required scope of work. Maintain valid registration with the Ontario College of Pharmacists throughout the term of employment, with no restrictions on pharmaceutical practice and no active disciplinary proceedings. Demonstrate expertise and assessment skills to elicit, interpret, and utilize patient information to support clinical decision-making. Demonstrate leadership skills to coordinate and deliver pharmaceutical services, support quality improvement initiatives, and collaborate effectively with healthcare professionals, partners, and internal and external stakeholders. Possess knowledge of professional practice standards, best practice guidelines, pharmaceutical theories, practices, and applicable legislation, including but not limited to the Health Care Consent Act, Personal Health Information Protection Act (PHIPA), Regulated Health Professions Act (RHPA), Ontario Hospitals Act, Child and Family Services Act (CFSA), Youth Criminal Justice Act (YCJA), and other relevant legislation. Possess knowledge of illness and disease theory, health promotion, disease prevention, pharmaceuticals, diagnostic and laboratory testing, evidence-based practice, and current treatment modalities. Demonstrate knowledge of mental health and addiction treatment options, with experience working with individuals experiencing addictions. Have experience working with individuals experiencing mental health concerns. Have experience providing pharmaceutical care to diverse client populations, including youth and adults. Have experience providing pharmaceutical care to marginalized and diverse populations, including individuals who have experienced trauma.
