About the role
Your Opportunity: We are currently recruiting two full-time Social Workers to join our Interprofessional Primary Care team. The Primary Care Social Worker is part of an interprofessional health team, providing biopsychosocial assessments and interventions for diverse populations. This role focuses on improving overall well-being by addressing social determinants of health, mental health concerns, and system navigation challenges. The Social Worker collaborates closely with patients, primary care providers, and community partners to enhance access to comprehensive, patient-centered care.
A Successful Candidate in This Role Will Have: Bachelor of Social Work (BSW) required; Master of Social Work (MSW) preferred. Membership in good standing with the Ontario College of Social Work and Social Service Workers. Possess professional liability insurance. Experience providing mental health and social work services in health care settings. Background supporting child, youth and families experiencing mental health and system barriers preferred. Experience working with multiple partners in Hamilton or other communities. Strong knowledge of social determinants of health and their impact on patient well-being. Experience working in interprofessional primary care teams. Health promotion experience preferred. Knowledge of relevant legislation (e.g., PHIPA, Mental Health Act, Child & Family Services Act). Strong understanding of Social Work Values and Ethics in clinical practice within healthcare settings.
Duties and Responsibilities: Conduct biopsychosocial assessments. Deliver evidence-based, patient-centered, short-term interventions for individuals, families, and groups. Develop care plans with patients, primary care providers, and interprofessional team members. Facilitate referrals to appropriate community and health services as needed. Provide case management as necessary. Support health promotion efforts to enhance outreach and patient outcomes. Develop and implement strategies to address the social determinants of health. Engage in outreach efforts to improve access to services for vulnerable populations. Work collaboratively with interprofessional health teams to optimize patient care. Assist patients in navigating complex health and social service systems, advocating to remove barriers to care. Support patient access to financial assistance programs, housing supports, food security initiatives, and employment resources. Educate team members and students on social work’s role and psychosocial patient issues. Staff are encouraged to act as a Field Instructor for social work students after one year of service (and five years of professional experience). Stay current with best practices, research, and legislation impacting social work in healthcare. Contribute to the development and evaluation of programs that enhance patient outcomes. Participate in team meetings, case discussions, and collaborative care planning. Engage in quality improvement initiatives to refine service delivery models. ***The above responsibilities are not to be considered all-inclusive.

In a FHT (Family Health Team) model, family doctors work closely with nurses, mental health counsellors, dietitians, psychiatrists, pharmacists and more to provide team-based care to patients.
