About the role:
The Hospital strives to deliver care that is centred on patients and families. Patient and Family Centred Care (PFCC)
is the provision of care that is respectful of, and responsive to individual patient/family preferences, needs, and values
and ensures that those values guide all decisions. Creating a caring environment for patients and families ensures
continuity of care and fosters a learning environment. Applicants are required to have a demonstrated knowledge,
understanding and commitment to this care philosophy.
The Nurse Practitioner (NP) in the Cardiovascular Services reports to the Director of the Cardiovascular, Medicine
and Renal Services, with a matrix reporting relationship for nursing practice to the Director of Nursing. The NP will
have a professional accountability for quality and safety to the Medical Advisory Committee through the Chief of
Departments and matrix relationship to the Chief Nursing Executive. The NP is an expert practitioner with a specialty
based focus who functions with a high level of autonomy, with advanced knowledge and decision making to
formulate clinical decisions to manage the health care of infants and children through an interdisciplinary team
approach. Nurse Practitioner practice address the five domains of Advance Practice Nursing: Clinical Practice,
Collaboration, Research, Leadership and Change Agent. The percentage of time dedicated to each of these domains
is variable, based on the program needs and will evolve with the program
The Cardiovascular Services Nurse Practitioner's primary focus is on the provision and coordination of health care for
patients with cardiovascular related diagnoses. This includes but is not limited to cardiology patients (Coronary Artery
Disease, Acute Coronary Syndrome, and Heart Failure) and vascular patients (Complex chronic wounds with a focus
on limb preservation, peripheral vascular disease). Practice will include participating in the coordination of care
services for medically complex patients, as well as conducting physical assessments, medical care planning and
providing health education for outpatient clinics and patient's admitted to Thunder Bay Regional Health Sciences
Centre.
EMPLOYMENT REQUIREMENTS:
Education/Experience:
* Unencumbered registration as a Registered Nurse in the Extended Class with the College of Nurses of Ontario
* Nurse Practitioner certification in Primary Health Care, Cardiovascular speciality preferred
* Successful completion of Graduate Studies
* Membership with RNAO or NPAO is required
Skills/Abilities:
* Minimum of 3 years recent Nurse Practitioner experience during the last 5 years, cardiovascular focus preferred
* Experience working with Cardiology and Vascular Patients
* Experience working with chronic, complex wounds with a focus on Limb Preservation
Assets:
* Demonstrates commitment to the principals of Patient and Family Centered Care
* Regular and sustained attendance as per hospital standard
* Knowledge and adherence to the hospital's Code of Conduct
Salary Range: $65.210 to 70.750
