As an Acute Care at Home Nurse, you will work closely with physicians, nurse practitioners, and the hospital team to deliver advanced nursing care to acute-level patients in their homes.
Work Assignment: Acute Care at Home Nurse (RN)
Status: Full Time, Part Time

Join CBI Home Heath’s Nursing team and bring acute care directly to patients in the comfort of their homes! As a Registered Nurse, you will leverage your clinical expertise to provide high quality, hospital-level care in a home setting. This role is ideal for nurses who thrive in a dynamic environment, can work autonomously, and are passionate about patient-centered care.
Key Responsibilities
- Conducting comprehensive nursing assessments and developing customized care plans
- Administering and managing infusion therapy, including medication administration and fluid management
- Performing peripheral venous access device (PVAD) insertion and maintenance
- Providing central venous access device (CVAD) care, including dressing changes and line maintenance
- Performing venipuncture for lab testing and ensuring timely specimen collection
- Overseeing medication management, promoting adherence to the patient’s medication regimen
- Providing wound care, including comprehensive assessments and dressing changes
- Implementing infection control measures to prevent the transmission of pathogens
- Collaborating with an interdisciplinary team, including physicians, nurse practitioners, and other healthcare professionals, to optimize patient well-being and health outcomes
- Educating patients and families on disease management, medications, and self-management strategies
- Utilizing point of care labs/testing in-home and remote patient monitoring (RPM) tools to help evaluate patient progress and respond to changes in conditions
- Documenting assessment and interventions using various technology /programs/apps
- Delivering evidence-based nursing care that promotes recovery, healing and optimal well-being
What You Need To Be Successful
- Registered Nurse with the College of Nurses of Ontario, in good standing
- 2 years of acute care experience (ICU, ED, medical-surgical or equivalent) preferred
- Previous home care/ community nursing experience preferred
- A clear criminal record check with vulnerable sector screening
- Proof of negative TB test and professional liability insurance
- Fully vaccinated against COVID-19 and provide proof of vaccination upon hire
- Strong critical thinking, nursing assessment and decision-making skills
- Strong verbal and written English communication skills, bilingual in French an asset
- Ability to travel independently within the community – a valid driver’s license, vehicle and vehicle insurance required
- Proficiency in IV therapy, oxygen therapy, venipuncture, and advanced clinical assessment
- A commitment to continuing education and willingness to actively participate in departmental educational activities
- Knowledge of the nursing process and the ability to apply this process to the practice of Nursing
- Familiarity and comfort utilizing technology (e.g. programs/apps) and mobile devices at point of care
- A commitment to evidence-informed practice and quality service delivery
- Ability to work effectively as an active team member of an interdisciplinary health care team
- A professional and respectful attitude to patients, families, the public and colleagues
