Position Overview
Are you a compassionate Licensed Practical Nurse looking to make a real difference? At Nichol Place, a 48-unit Assisted Living Program supporting adults with mental health and substance use challenges, you will provide person-centered care, support recovery, and promote independence in a collaborative, team-oriented environment.
Reporting to the Program Manager, the successful candidate will provide direct nursing care to residents, including medication administration, treatments, and health monitoring according to care plans and physician orders. They will conduct regular mental health assessments and support residents in achieving rehabilitation goals that promote independence. The LPN will collaborate with an interdisciplinary team to develop and evaluate individualized care plans while accurately documenting progress. Additional responsibilities include responding to mental health crises and medical emergencies, educating residents on medication and self-care, and fostering a safe, supportive environment for individuals with diverse mental health challenges. The role also involves participation in team meetings and quality improvement initiatives, and may occasionally require working at other Assisted Living sites as needed.
Qualifications
The ideal candidate will have graduated from an approved Practical Nursing Program and must hold current, active LPN registration in good standing, along with a minimum of one year of experience in mental health, assisted living, rehabilitation, or related care settings. Current CPR/First Aid certification is also required.
Candidates should possess broad knowledge of practical nursing, trauma-informed care, mental health, substance use, and concurrent disorders. Strong skills in medication management, recovery-oriented practices, care planning, leadership, and communication are essential. The ideal candidate excels in both team-based and independent settings, demonstrates strong assessment, critical thinking, and clinical decision-making abilities, and is experienced in harm reduction, community resources, group facilitation, and health education.

The schedule of this position will be as follows:
- Saturday-Monday- 8:00AM- 4:00PM
- Tuesday-Wednesday- 3:00PM- 11:00PM
This role requires the incumbent to undergo a Police Information Check (PIC), and a Vulnerable Sector Check by the Ministry of Justice.
