HOURS OF WORK:
The stated hours of work are approximate as schedules are subject to change with the operational needs of the Health Centre. Changes made to hours of work will follow the terms of the applicable collective agreement, company policy and/or legislation.
WHAT YOU’LL DO:
The ADEC registered nurse, working with an interdisciplinary team, helps adult clients with chronic conditions including diabetes by supporting collaborative care with a focus on self-management. Team case management and patient education, involving the patient’s families and support persons, helps support a holistic approach to better manage chronic disease and promote treatment commitment. The RN helps to address the clinical and psychosocial aspects of diabetes that impact the burden of disease.
WHAT YOU’LL BRING:
- Registered Nurse with current Certificate of Competence issued by the College of Nurses of Ontario
- Current Certification in Diabetes Education (CDE) required
- Completion of the Diabetes Educator Certificate Program or equivalent preferred.

- Demonstrated ability to work as a member of a health care team.
- Excellent interpersonal, written, and verbal communication skills.
- Demonstrated ability to work effectively with patients, families, physicians, and health centre staff.
- Demonstrated ability to build and maintain effective relationships with a diverse range of people, and work effectively within an interprofessional team
- Demonstrated ability to organize workload and establish priorities effectively;
- Demonstrated problem-solving skills and ability to exercise independent judgement and to assess situations and address problems efficiently and effectively; and
- Motivated self-directed learner, who take initiative and seeks learning opportunities to enhance client care and professional competence.
- Ability to work independently and use initiative and good judgement when performing duties.
- Excellent problem-solving skills.
- Demonstrated ability to maintain confidentiality.
- Possess a passion for diabetes care and will champion the ongoing development of this chronic disease management initiative in our community.
- Excellent assessment, planning and organizational skills.
- Knowledge of the principles of adult learning.
- Demonstrated ability to facilitate both individual and group educational sessions using the principles of education.
- Flexibility to meet individual client demands.
- Good customer service skills.
- Basic computer skills.
- Demonstrated commitment to continued education.
- Membership in Diabetes Education Section of Canadian Diabetes Association preferred.
- Two years’ clinical experience in management of diabetes preferred.
- Experience using Insulin Pumps from a variety of vendors preferred.
- Experience working with Epic electronic health record preferred.
