Bilingual Registered Practical Nurse
Casual Onsite Position
South-East Ottawa Community Health Centre (SEOCHC) is a non-profit, community-governed organization that provides a range of multi-disciplinary services to clients, including primary care, health promotion, social services, community and resource development, home support, and advocacy to address the social determinants of health. SEOCHC particularly responds to people who face barriers to accessing health and social services and who live in Southeast Ottawa. Our services and programs are designed to meet the needs of our community.
At SEOCHC, we strive to ensure that our team reflects the communities we serve. We work within an anti-racist, anti-oppressive framework that acknowledges the existence and impacts of systemic racism and seeks to ensure that systems are fair and equitable for everyone. Our primary Care clinic is seeking a compassionate and dedicated Registered Practical Nurse (RPN) to join our interprofessional primary care team. The RPN will work closely with physicians, nurse practitioners, registered nurses, social workers, and other allied health professionals to deliver high-quality, evidence-based care to clients across the lifespan.
Responsibilities

Provide comprehensive nursing care within the RPN scope of practice, including health assessments, immunizations, wound care, chronic disease management, and medication administration.

Support clinical procedures and assist in minor medical interventions.

Collaborate with the healthcare team to develop, implement, and evaluate individualized care plans.

Promote health education and self-management strategies to clients and families.

Maintain accurate and timely documentation in the electronic medical record (EMR).

Participate in quality improvement initiatives and team meetings.

Reach out to the client to assess the urgent situation and determine the most appropriate course of action in collaboration with the on-duty physician.
Qualifications
-
Registered Practical Nurse (RPN) in good standing with the College of Nurses of Ontario (CNO).

Minimum 2-3 years of experience in primary care, community health, or related setting preferred.

Experience working with marginalized populations, including individuals with complex health and social needs.

Proficiency with electronic medical records (EMRs), preferably PS Suite.

Fluency in English and French language preferred; other languages representing our community (Arabic, Somali) are a strong asset.

Ability to work flexible hours, including evenings and weekends.
Wage range: $26.32/hour - $30.96/hour

We serve the constituency of South-East Ottawa, and particularly try to meet the needs of folks with limited access to health and social supports, including newcomers to Canada. Most of our services are free and many of our programs are operated both at our main location (1355 Bank St.) and out in the community. Our second site is EarlyON which serves families in the Heatherington-Fairlea community and our third site is at the Hunt Club-Riverside Community Centre. Programs include drop-ins for breastfeeding, exercises for seniors and everything in between. Check us out at www.seochc.on.ca
