Position Summary:

  • Responsible for development of nursing care plans and counselling for patients with diabetes and/or co-morbidities such as renal, cardiovascular disease and mental illness
  • Responsible and accountable for decisions and actions in the provision and management of nursing care for patients with diabetes in a collaborative inter-professional team environment
  • Broad understanding of resources available in the community to help support caseload of patients through critical thinking and problem solving to ensure best practices are maintained
  • Assist and support clients to gain the knowledge, skills, attitude and self-awareness necessary to effectively manage his/her diabetes
  • Monthly rotations in the inpatient units at the Mississauga Hospital to support bedside teaching for diabetes and safe discharge.
  • Function as a part of a multidisciplinary team, and act as a resource for other health care providers and the community
  • Using a variety of teaching methods, incorporating the principles of adult education and a variety of client engagement or empowerment modalities to facilitate behaviour change (ie motivational interviewing, stages of change )
  • Facilitates group education programs for patients as needed
  • Develops appropriate education materials to meet the needs of the clients
  • Assists in program planning and evaluation

Key Qualifications:

  • Certified Diabetes Educator
  • Membership in the Diabetes Educator Section of Diabetes Canada preferred
  • Minimum 5 years’ experience in counselling clients with diabetes on an individual and group basis
  • Good working knowledge of diabetes and comorbidities through the lifecycle, adult teaching principles and patient engagement strategies
  • Good working knowledge of diabetes medications, including insulin
  • Highly effective decision making, problem solving and organizational skills combined with excellent interpersonal and communication skills, both oral and written
  • Proficiency in computers, Microsoft Office and experience with electronic medical records and good keyboarding skills are essential
  • Reliable, flexible, creative professional, able to work effectively and collaboratively in a multidisciplinary team approach required
  • Experience using medical directives an asset
  • Additional language an asset
  • Positive attendance and work record essential
  • Member in good standing with College of Nurses of Ontario
