About the Dufferin HART Hub
The Dufferin HART Hub is dedicated to improving outcomes for individuals in Dufferin County experiencing homelessness, mental health challenges, and substance use issues. By providing integrated, low-barrier support services, we aim to enhance the quality of life, physical health, and overall well-being of individuals in our community. We take a person-centered, equity-focused approach to ensure that clients have access to the care they need when they need it.
Role Overview
We are seeking a dynamic and compassionate Service Navigator to be an integral part of the HART Hub Dufferin, providing support to a multidisciplinary team delivering comprehensive, integrated care services to vulnerable individuals.
Key Responsibilities:
Reporting to the Manager, HART Hub Dufferin, the Service Navigator, will be the first point of contact for clients seeking support through one of our programs.
They will operate within the HART Hub Dufferin sites that are focused on providing services to clients who are at risk of homelessness, addiction, recovery and treatment.
Duties and Responsibilities:
- Act as first point of contact for community clients within the HART Hub Dufferin
- Carry a caseload of clients in our second stage housing programs attached to HART Hub Dufferin and provide case management wrap around support
- Conduct screening/assessment interviews with community clients requesting services
- Develop a client centered case plan in collaboration with the client and any identified support
- Work to establish rapport and build client engagement with available supports
- Deliver brief interventions with clients during screening or assessment services
- Develop formal links and networks with other information response and referral type positions working with similar client groups
- Coordinate services within the multidisciplinary team at HART Hub Dufferin
- Make appropriate external referrals as per the client centered case plan
- Keep up to date with knowledge of external community resources and internal SHIP resources
- Ensure adherence to policies, procedures and collective agreement
- Work collaboratively with partnership agencies to promote the services offered at HART Hub Dufferin and establish referral pathways
- Work independently and collaboratively with a multidisciplinary team to carry out recovery and support
- Understand and be able to integrate a philosophy of client centred care into daily responsibilities
- Demonstrate a strong commitment to quality care and service delivery based on SHIP’s Mission and Values
- Ensure appropriate records are kept and make sure that statistical information is maintained as per agency standards
- Adhere to agency Health & Safety Program and the Client Safety Framework
- Perform additional duties as required/delegated by Management
Qualifications:
- Bachelor's Degree or Diploma in Social Work, Sociology or Health Services
- At least 3 years’ experience working across a continuum of community services in Dufferin County
- Demonstrated experience and expertise in interviewing and assessing client needs
- Experience in the application of a variety of screening and assessment procedures and tools
- Experience providing short- or long-term case management supports
- Excellent oral and written communication skills are essential
- Bilingual candidates are encouraged to apply
- Knowledge of MS Suite (Word, Excel, Power Point and Outlook)
- A vehicle, valid driver’s license, and adequate insurance coverage
- Valid Certification in First Aid & CPR
- Training in ASIST, CPI and Naloxone administration
Reporting Function:
The Service Navigator will report directly to the HART Hub Dufferin Manager. They will work with both internal and external partners and will interact with all levels within the organization. They will have no supervisory responsibilities.
They will be a support system, resource, and an advocator for the team and Company.
Working Conditions and Physical Requirements:
- Hours are typically from Monday – Friday, 8:30 am to 4:30 pm but may vary depending on client needs
- Average amount of physical effort is required.
- Able to sit/stand for prolonged periods of time
- Able to answer and communicate telephone calls and general computer use
- Able to lift up to 20lbs (product, documents, etc.)
- Vehicle travel to sites for visits/walk-throughs and open doors as required.
