How You Will Make an Impact

The Registered Social Worker (RSW) requires adherence to the mission and values of Good Shepherd Centres and compliance with the Code of Ethics and Standards of Practice of the Ontario College of Social Workers and Social Service Workers (OCSWSSW). The role emphasizes delivering services in a dignified, culturally competent, and respectful manner, incorporating an anti-racism and anti-oppression framework. The individual is expected to demonstrate a strong work ethic, leadership, and role modeling of behaviors aligned with the organization's mission, vision, and values.

The position involves providing psychosocial grief and bereavement support to residents, families, and staff in consultation with the Director and Clinical Manager of Seniors, Hospice & Palliative Care, and the interdisciplinary team. The individual plays a critical role in the hospice care support team, collaborating with internal and external partners, and is responsible for data collection related to their functions, supporting research and quality improvement initiatives.

Responsibilities include conducting psychosocial assessments, providing counseling to residents and families, connecting families to community supports and coordinating hospice care family meetings. The role also encompasses responding to inquiries about hospice, grief, and bereavement support, developing and maintaining relationships with community partners, and participating in interdisciplinary team conferences to formulate and coordinate comprehensive treatment programs.

Reporting to the Clinical Manager, Hospice & Palliative Care, Good Shepherd Centres, the Registered Social Worker (RSW) enhances the dignity and quality of life of our clients, visitors and staff by providing direct services to clients. This includes liaising with internal and external community service providers, primary care, hospital partners, Good Shepherd Centres & Not for Profit Homes and Ontario Health @Home. All duties are performed within a multidisciplinary team.

Assists with legal matters such as Power of Attorney (POA) kits, engages in evidence-based research to improve psychosocial interventions and outcomes, and continuously monitors internal and external factors impacting care. Adherence to relevant legislation, regulations, and standards set by the OCSWSSW and Good Shepherd Centers is essential.

Additionally, the individual would observe all Good Shepherd health and safety rules and work in compliance with the provisions of the Occupational Health and Safety Act and its regulations

What You'll Bring to Our Team

  • Minimum Master of Social Work (MSW) degree and in good standing certificate with OCSWSSW.
  • 5+ years' experience in a Registered Social Worker role, including, counselling (individual, family) and service system navigation.
  • Minimum two (2) years' experience working in and environment providing support to persons at end of life; receiving palliative or hospice care.
  • Minimum two (2) years' experience in a healthcare setting or home and community care with specialized knowledge of grief and bereavement across the lifespan, advance care planning, geriatrics, dementia care, and mental health.
  • Certifications in individual and group evidenced based interventions (e.g. motivational interviewing CBT, DBT, ACT)
  • Strong understanding of anti-oppression and trauma-informed care.
  • The candidate should have experience with clients in community settings, an excellent understanding of harm reduction for drug users, and knowledge of issues affecting older adults, such as chronic illnesses, caregiver stress, abuse, and mental health.
  • In-depth knowledge of Ontario legislation related to substitute decisions, POA, and the Public Guardian of Ontario is essential, along with familiarity with long-term care and home/community care services.
  • The candidate should have strong social work assessment, counseling, and case management skills, experience with diverse populations including Indigenous, Black, racialized, and LGBTQ2 communities, and the ability to identify and address systemic inequities.
  • Excellent knowledge of community resources, social determinants of health, and evidence-informed practices for older adults is required, along with the capacity to develop and facilitate group sessions for emotional and behavioral management skills.
  • The candidate must possess excellent assessment, evaluation, judgment, and decision-making skills, with the ability to plan, organize, and implement both proactively and responsively.
  • Familiarity with multi-disciplinary team dynamics, effective teamwork skills, and the capacity to work in such settings are essential.
  • Demonstrate high emotional intelligence, including self-awareness, reflection, and the ability to manage stress in emotionally sensitive situations.
  • Knowledge and sensitivity to religious, ethnic, and cultural diversity are crucial. Excellent organizational skills are needed to meet client demands in a dynamic environment, along with strong leadership skills to collaborate, problem-solve, and effectively serve clients.
  • Current CPR and First Aid certificate an asset
  • Non-Violent Crisis Intervention certification an asset
  • Communication ability in a language other than English an asset
  • Observe all health and safety rules and work in compliance with the provisions of the occupational Health and Safety Act and its regulations.
