Provide family-centered care to children with immune deficiency illnesses and severe allergies. Focus on remote care management, care coordination, case management, and patient education.

We have an exciting opportunity to join the Ambulatory Care Nursing Team in the Immunology Allergy Clinic here at SickKids. As a Registered Nurse, you will provide family-centered nursing care to children, as you assess patient needs, support interventions and evaluate the response of treatment for kidney related illnesses. Ambulatory care offers a unique nursing experience focused on telehealth, care coordination, case management and caregiver/patient education. It requires strong clinical questioning & assessment skills along with critical thinking to ensure the timely support of complex and diverse nephrology patients of SickKids

Here's What You'll Get To Do

  • Develop content expertise: Gain foundational knowledge related to immunology and allergy including but not limited to understanding immune deficiencies of various origins, allergy testing, challenges, desensitization and the evolving field of immunotherapy treatments, patient management and evidence-based practice to ensure the most up to date treatment modalities.
  • Clinical Assessment: Assess and monitor patients' health status, particularly related to immune deficiencies and allergy challenges and immunotherapy, both in person and through telehealth methods (phone, email, mychart, remote care management of food immunotherapy treatment).
  • Care Coordination: Organize and manage patient care activities, including referrals, tests, and follow-ups, liaise between patient/caregivers and the interprofessional care team to provide care/call back instructions and support administrative tasks (ex: funding applications & MOH forms).
  • Patient Education: Educate patients and families about health conditions, trajectory of illness, treatments (including medications and oral challenge dosing), and wellness strategies tailored to a patient's health literacy, educational & cultural needs in a developmentally appropriate way.
  • Virtual Care Support: Use telehealth technologies (phone, email, video calls, myChart) to provide remote care and triage patients. This is a fundamental aspect of the role, ensuring timely access to the appropriate level of care by using clinical questioning, clinical judgement, clinical algorithms, protocols, or guidelines to systematically assess patient needs and symptoms.

Here's What You'll Need

  • Graduate of an approved Baccalaureate Nursing Program
  • Registration with the College of Nurses of Ontario as a Registered Nurse in good standing.
  • 2-3 years of nursing experience (pediatrics, ambulatory care and/or immunology allergy experience preferred)
  • Experience with Epic is an asset
  • Strong communication, critical thinking, and teamwork skills required
  • Strong problem-solving and decision-making skills to support complex patient care.
  • Excellent communication skills to effectively interact with patients, families, and healthcare teams in verbal and written forms.
  • Ability to work collaboratively with interdisciplinary teams to provide comprehensive care.
  • Flexibility to adapt to changing patient needs and healthcare environments.
  • Ability to make independent decisions and take actions based on their professional knowledge, skills, and judgment without direct supervision to meet patient needs in the dynamic nature of Ambulatory Care [Clinical decision-making, self-management, accountability, & professional confidence]
  • Commitment to lifelong learning and staying updated with the latest nursing practices and research.

Employment Type : Part-Time with benefits, [0.6 FTE permanent, 0.8 FTE for 1 year, 0.2 FTE 1 year contract]
