Job Description: POSITION PURPOSE SUMMARY: The Registered Practical Nurse (RPN) provides safe, competent, and person-centered nursing care within the scope of RPN practice under the direction of the Director of Care and the Registered Nurse. The role contributes to the broader mission and values of The Salvation Army, upholding standards of service that reflect its spiritual and social commitment to the community. ACCOUNTABILITIES: Incumbents may perform all, some, or similar range of duties as below. - Deliver safe, effective, and person-centered nursing care in accordance with the RPN scope of practice, care plans, and regulatory standards. - Administer medications and treatments accurately, document and report client responses and changes in condition in a timely and thorough manner. - Support assessment, planning, implementation, and evaluation of individualized care plans collaboratively with clients, families, and the interdisciplinary team. - Maintain complete and accurate clinical documentation using approved electronic health record systems and medication administration records. - Facilitate clear communication with clients, families, physicians, and team members to ensure informed, respectful, and coordinated care as directed. - Participate in staff orientation, mentoring, education, and performance feedback as assigned by nursing leadership. - Promote infection prevention, health and safety, and emergency protocols to maintain a safe and clean environment for clients and staff. - May coordinate and support Personal Support Workers (PSWs)/Health Care Aides to ensure clear direction and quality of care. - May act as shift lead when assigned by leadership, coordinating workflow, resource allocation, and care delivery. - Uphold organizational policies, professional ethics, and Ministry of Health requirements to support quality improvement and regulatory compliance. - Represent the organization in a respectful and professional manner, reflecting the mission and values of The Salvation Army. - Other related duties as assigned. CRITICAL RELATIONSHIP MANAGEMENT: Internal: Ministry Unit leadership, staff, and volunteers. External: External agencies (e.g., EMS, health care service agencies and providers), clients and clients’ family members of ministry unit in person, students. MANAGERIAL RESPONSIBILITIES: Reports directly to the ADOC WORKING CONDITIONS: - Work is primarily performed indoors within the facility in a fast-paced environment. - This work requires sitting, standing, walking, and keyboarding. - Some scheduled overtime and shift/weekend work may be required for this position. - Occasional travel within the city may be associated with this position. A valid driver's license and clear driver's abstract will be required. - The work environment is generally positive; however, the incumbent may occasionally interact with individuals displaying challenging behaviours. EDUCATION AND EXPERIENCE QUALIFICATIONS: Education, Qualifications and Certifications: - Graduate of an approved Practical Nurse’s Program. - Certification as required within the scope of the assigned position in alignment with provincial legislation. - Current certification in First Aide & CPR Experience and Knowledge Requirements: - Three years of related experience is required. - Knowledge of Nursing standards/protocols and basic nursing care procedures is required in area of nursing specialty related to the job assignment. - Proficiency in MS office. Skills and Capabilities: - Willingness to develop an understanding and support for the mission and purpose of The Salvation Army in Canada and its implications as related to position responsibilities. - Work in compliance with OH&S Act and Regulations and abide by The Salvation Army’s health and safety policies and procedures. - Excellent interpersonal and communication skills, including active listening, empathy, and the ability to work effectively with staff, clients, families, volunteers, and visitors. - Demonstrated commitment to client-centered care, treating others with respect and dignity. - Demonstrated patience, understanding, and professionalism in all interactions. - Demonstrated attention to detail with good analytical and problem-solving skills. - Keen observation and critical thinking skills. - Strong oral and written communication skills. - Strong organizational skills. - Self-motivated, disciplined, and adaptable, with a willingness to learn and embrace change. - Ability to work cooperatively and effectively with staff, clients, visitors, and the public. - Ability to achieve objectives, ensuring multiple priorities and tasks are appropriately carried out. - High level of integrity, confidentiality, and professional ethics, with a balanced sense of fairness and flexibility. - Interest in participating in professional development activities. Requirements: Current registration as a Registered Practical Nurse (RPN) in good standing with the College of Nurses of Ontario (CNO). Current CPR Certification (Basic Life Support/CPR-C or equivalent). Vulnerable Sector Check (VSC) completed within the last six (6) months and satisfactory to the employer. Negative TB screening requirements met, including a chest X-ray completed within the last one (1) year if indicated. Previous Long-Term Care (LTC) experience preferred. Demonstrated knowledge of applicable legislation, including the Fixing Long-Term Care Act, 2021. Strong assessment, communication, documentation, and leadership skills.
