One of a network of provincial community health centres, the NorWest Community Health Centres is a community based, non-profit organization funded by Ontario Health, emphasizing access to primary health care and prevention programs and services.
We are currently seeking a 1 FTE Registered Nurse, as a valuable member of the Community Health Centres’ team who will be an effective, collaborative member of NorWest CHC providing clinical services to clients who face barriers to accessing care (under housed, living with mental health challenges or diagnoses, current or past substance use, complex health histories, 2SLGBTQIA+, newcomers, non-insured). This is a mobile position that will provide clinical care, illness prevention and health promotion in social housing units and other designated areas while collaborating with external partners.
The position has a competitive salary, an excellent group health benefit package and the Health Care of Ontario Pension Plan is an optional benefit.
QUALIFICATIONS:
- BScN degree with current CNO registration and 2-3 years nursing experience.
- Ability to work flexible hours including evenings and weekends.
- Ability to travel for work.
- Strong organizational, communication and leadership skills.
- Strong ability to think critically and problem solve creatively.
- Ability to work both independently, and collaboratively, within interdisciplinary team environment.
- Awareness and sensitivity to socio-economic and cultural issues.
- Awareness of and sensitivity to health issues particularly within populations who face barriers to accessing care (under housed, living with mental health challenges or diagnoses, current or past substance use, complex health histories, 2SLGBTQI+, newcomers, non-insured).
- Perform clinical activities according to the College of Nurses standards for nursing practice.
- Utilizes professional assessment skills to identify physical, mental, and psychosocial needs of clients.
- Knowledge and proficiency in current evidence based practices of primary care delivery, with an emphasis on health promotion and harm reduction.
- Knowledge of virtual care system to support the clinical team by planning for, organizing and providing virtual care services to clients and staff.
- Proficiency in electronic charting.
- Valid driver’s license, access to a vehicle and ability to travel to other service locations.
