Job Summary
Social Workers work in partnership with individuals, families and groups to access services, co-create ideas to enable clients to find effective solutions to problems and improve their quality of life. Social Workers offer a distinctive role in resolving barriers to service utilization and to helping families negotiate the system. The proportion of time spent in various social work functions will vary according to the area of service assigned.
Key Responsibilities

Assists in resident admissions process, including the completion of relevant admission assessments
-
Provides resident and family psychosocial assessments, support and counseling
-
In collaboration with the inter-professional team, implements a resident centred care plan and coordinates the discharge planning process for residents
-
Coordinates care conferences with members of the inter-professional team, residents and families
-
Supports resident advisory council, as appropriate
-
Provides support/consultation to other disciplines on the team re: psychosocial issues particularly related to complex patient/family situations
-
Crisis intervention
-
Acts as a linkage and communication conduit between the family and the care team
-
Establishes community linkages and outreach with relevant agencies and other Baycrest programs and services
-
Participates in teaching and other student supervision activities for health professional students assigned to this unit
Qualifications
-
Masters of Social Work degree
-
Current and valid registration with the Ontario College of Social Workers and Social Service Workers (‘OCSWSSW’)
-
Knowledge and understanding of ALC practices and principles required
-
Experience in discharge planning and system navigation preferred
-
Background in Minimum Data Set (‘MDS’) assessment and understanding of derived clinical outcomes preferred
-
Excellent interpersonal, communication and problem solving skills
-
Basic computer skills – Meditech, Microsoft Word and Excel, RMNR preferred
