General Responsibilities

The HHP Team Lead supports the efficient and effective operations of HHP services and staff. Supervises the work of the Homeless Health Program, including both in-shelter and outreach interprofessional teams.

Accountability

  • The Team Lead (Nursing) reports to the Manager of the appropriate program

Specific responsibilities

  • Reporting to the Manager – Clinical Community Care, the HHP Team Lead supports the efficient and effective operations of HHP services and staff.
  • Supervises the work of the Homeless Health Program, including both in-shelter and outreach interprofessional teams, according to planning guidelines and operational policies of the Homeless Health program and WellFort Community Health Services.
  • Acts as a professional role model, mentor and change agent for staff by demonstrating clinical expertise and evidence of ongoing professional development.
  • Works with team members and Management to ensure adequate staffing requirements are met for both in-shelter and community outreach model, including creation of draft schedules to be reviewed and approved by leadership in alignment with the employment standards act and WellFort policies.
  • Holds regular team meetings and huddles to keep team members appraised of Key Performance Indicators (KPI’s) and information necessary for excellent service delivery and team effectiveness.
  • Provides clinical leadership and direction by ensuring patient care is delivered according to professional nursing standards and in accordance with policies and procedures.
  • In collaboration with the service delivery team, contributes to the delivery of the client’s care plan.
  • Works under delegated acts as necessary with primary care team.
  • Partners across other service providers external to WellFort to coordinate and support integrated care.
  • Documents problems identified using a specified framework.
  • Selects and implements strategies or interventions from established care plans in consultation with client and/or caregiver and health care delivery team.
  • Utilizes adult learning principles and established training materials to teach and enhance skill levels of service providers.
  • Meets with individual team members to build education and learning plans and provides input to annual PDP.
  • Acts as clinical resource to the service delivery team.
  • Demonstrates effective time management, planning and organization of day’s work activities and of team members.
  • Works collaboratively with clinical and non-clinical staff, allied health, community partners across the Peel shelter system, and other relevant partners in the advancement of the care model; ensures role clarity in the different functions of clinical and nonclinical staff.
  • Participates in staff planning and orientation of new staff, including EMR training.
  • Responsible for first line risk mitigation strategies for client or staff incidents.
  • Supports the building of an integrated team of both WellFort CHS and other Community Services staff working as one team.
  • Bridges programming between the Safe Consumption Site, Bloom program and Homeless Health programming as well as the comprehensive WellFort CHS services to support an integrated model of care.

Qualifications

  • Diploma or degree in nursing from a recognized community college or university. Baccalaureate degree an asset.
  • Registration with the College of Nurses of Ontario and in good standing.
  • Nursing experience in a variety of care settings one of which must include community health, primary care and structurally vulnerable and homeless care.
  • Team Lead or supervisory experienced preferred.
  • Experience working in a Safe Consumption Site or Homeless shelter environment in the past is preferred.
  • Coaching, mentoring and conflict resolution skills.
  • Experience working with communities and individuals who face barriers accessing tradition health care institutions such as poverty, discrimination, mental health and substance use issues.
  • Program planning experience considered an asset.
  • An understanding of and critical analysis of the determinants of health and social factors, such as power dynamics that affect individual, organizational and community health.
  • Excellent written communication and oral skills that meet the needs of the individual and/or community at the appropriate literacy level.
  • Experience working with confidential and time sensitive documents and information.
  • Excellent interpersonal skills and the ability to multitask in a fast paced environment.
  • Experience working in a value based non-profit or social services organization.
  • Demonstrated ability to work independently and interdependently in an interdisciplinary team environment in which the skills and responsibilities of each team member are recognized and respected.
  • Demonstrated proficiency in the use of computers and various computer software applications, such as Electronic Medical Record (EMR).
  • Languages relevant to Peel Region residents is a definite asset.

Requirements:

  • Valid First Aid & CPR Certification
  • Must present a Vulnerable Sector Check that meets the employer’s standard
  • Completed 2-Step TB Test
  • Must be available to work weekends and statutory holidays
