I’m recruiting on behalf of a valued client in the long-term care space who’s looking for a dedicated and compassionate Nurse Practitioner to join their team at a reputable LTC facility in Toronto’s West Park area.

What You’ll Be Doing:

Deliver primary care and chronic disease management in a long-term care setting.

Support dementia care, palliative care, and behavioral interventions.

Collaborate closely with nurses, PSWs, social workers, and families.

Develop individualized care plans and support transitions from hospital to home.

Provide occasional after-hours or on-call coverage as needed.

Educate, mentor, and bring leadership to the clinical team.
What We’re Looking For:

Registered Nurse Practitioner (CNO – Extended Class).
-
Ideally 3+ years of NP experience (strong clinical reasoning is a must).

Experience in long-term care or with geriatric populations is a big plus.

Strong assessment, communication, and problem-solving skills.

Open availability
