Reporting Relationship
The Elders Outreach Worker will report directly to the Home Care Program Manager of M’Chigeeng Health Services; maintain communication within the department and internal working relationships with all Departments within M’Chigeeng First Nation.
Position Summary
The Elders Outreach Worker is responsible to provide recreational, educational and community support services to the elderly community members of M’Chigeeng First Nation.
Essential Functions
-
Plan, organize and implement educational and social activities for the elderly members of our community.

Formulate concrete goals and objectives and identify appropriate internal and external resources to meet identified needs in the provision of support services to Elders of the community.

Incorporate and promote Anishnabek cultural values, beliefs, practices and Anishinabemowin into program delivery.
-
Ensure community members are informed of upcoming events and activities organized by the Home Care & Community Program.

Provide administrative reports in a timely manner, as required for funding and accountability.

Provide administrative duties in relation to the medical equipment loan program and ensure all equipment is maintained. Notify Program Manager of any equipment deficiencies.

Ensure Home & Community Care Program passenger bus is maintained according to Ministry of Transportation Standards.

Provide care and home support services as required for elderly residents and community band members.

Practice strict confidentiality, privacy, ethical and non-bias approaches.

Engage in regular liaison with the Home & Community Care Team, other related programs and/or departments within the Community and external agencies.

Participate in on-going professional development as it relates to the position.

Coordinate and participate in cultural events and ensure cultural components are incorporated and practiced in all areas of the Elders Program where possible.

Input support services into the electronic medical record management software system on a regular and consistent basis.

Will take an active role and participate in interdepartmental strategic planning activities by assisting in the formulation of annual program goals and objectives for the Home & Community Care Program.
Qualifications & Requirements
Minimum
-
OSSD with relevant Post Secondary level courses with a minimum of two (2) years’ work related experience

Health Care Aide Certificate
-
Personal Support Worker Certificate
-
Work experience with volunteers, developmentally challenged and the elderly.
Preferred
-
College Diploma in health care and/or social services related program with three (3) years adequate related work experience
Other Knowledge Skills & Abilities
-
Maintains and promotes strict confidentiality

Knowledge of community and external resources
-
Proficient computer skills
-
Excellent change management and relationship-building (partnering & networking) skills
-
Well organized with ability to multi-task, work well under pressure, and to prioritize work

Valid Class “G” Drivers License

Knowledge of First Nation issues, culture, traditions, customs and practices
-
Fluency in Anishinabemowin or a willingness to learn
-
Vulnerable Sector Screening (VSS) required (Current 30 days, original or certified copy)
CRC/VSS Rationale: M’Chigeeng First Nation and its agents are in a position of trust in services to band member children and/or vulnerable adults, so a safe and secure working environment is mandatory. Applicants must provide a clear VSS Certificate and the CRC must have a record clear and/or dated of any convictions relating to alcohol, drugs, assault, violence and theft. The CRC and VSS must be current (30 days) and must be an Original.
