About the role

Summary JOB SUMMARY:Within the context of a client and family centred model of care and, in accordance with the Mission, Vision and Values, and strategic directions of Providence Health Care, the person promotes a safe, respectful, and civil working environment for patients, residents, families, visitors and staff.Reporting to the Patient Care Manager or Resident Care Manager and working in collaboration with the interdisciplinary household team, the Household Licensed Practical Nurse (LPN) is responsible for providing nursing care to an assigned group of patients/residents according to established policies and procedures and the British Columbia College of Nurses and Midwives Standards for Licensed Practical Nurses. The Household LPN functions as an independent clinician, while recognizing situations such as changes in stability or predictability, or care that falls outside the scope or competence of the LPN, requiring consultation or referral to another health care team member including a Registered Nurse (RN) or Registered Psychiatric Nurse (RPN). The Household LPN assists residents in meeting their physical, social, spiritual, and psychological needs, consistent with the mission, vision and values of Providence Health Care (PHC) and established protocols. Participates in developing care plans, conducting assessments, implementing, and evaluating interventions, documenting information, and acting as an advocate for residents. The Household LPN is a member of the household team and works with the team to ensure that the abilities, preferences and routines of each resident are considered in decision making, and determining the order of the day. The Household LPN collaborates with other members of the household care team to share tasks and duties as appropriate to ensure the functioning of the household. Participates in a variety of meetings including care conferences and assists with orientation of new staff and students as required. Responds and reports incidents according to established procedures and performs clerical duties, as needed. QUALIFICATIONS:Education, Training and ExperienceGraduation from a recognized program for Practical Nurses or an equivalent combination of education, training and experience. Registration with the BC College of Nurses and Midwives (BCCNM) as a practicing LPN registrant. Current basic cardiac life support certification. Skills and AbilitiesDemonstrated knowledge and ability to perform Cardiopulmonary Resuscitation (CPR) procedures appropriate for the area of service.Demonstrated knowledge and competencies of a Licensed Practical Nurse as currently required by BCCNM. Demonstrated ability to communicate effectively, both verbally and in writing, with residents, families, co-workers, members of the multi-disciplinary care team and the public.Ability to deal effectively with others.Ability to organize work and implement a plan of care.Demonstrated ability to affectively model behaviours associated with the Home for Us model of care and mentor staff in the same.Ability to operate related equipment.Physical ability to carry out the duties of the position.Ability to work as a member in an interdisciplinary team.Ability to apply decision making and analytical skills.Ability to apply and role model conflict resolution strategies.Ability to prioritize and organize work.Ability to teach and identify resident learning needs.Direct Nursing Care:Provides safe and respectful nursing care to an assigned group of residents by following established guidelines, policies, and procedures, adapting to individualized care plans as they evolve, prioritizing resident needs, and aligning care with the principles of the Home for Us model of care.Performs nursing care and procedures and evaluates outcomes. Administers medications to assigned residents, according to established procedures, and monitors medication’s effects. Maintains communication with RN/RPN or other members of the interdisciplinary team as required.Conducts head-to-toe assessments, monitors, and recognizes abnormal signs or symptoms including laboratory values, and communicates with the RN/RPN, if warranted. Consults other members of the interdisciplinary care team as required. Supports and partners with the RN/RPN in the care of patients/residents with instability or unpredictable outcomes.Documents care provided, such as assessments, status, progress, treatments, changes in condition, medications administered and discharge planning, in paper and/or computerized environment.Uses evidence-based resources such as BCCNM practice standards, decision support tools, and drug reference guides, Learning Hub and other recommended continuing education, in-services, and workshops, to keep nursing knowledge and procedures up to date.Assists with the orientation of new staff and student preceptors by performing duties such as demonstrating work procedures, demonstrating proper use/location of equipment, sharing clinical knowledge, using effective communication, acting as a positive role model, and modeling the Home for Us Model of Care in their work.Assesses resident’s/family’s actual and potential strengths and limitations including physiological, psychological, socio-cultural, and spiritual needs. Documents these needs in the care plan and makes referrals to identified resources where appropriate. Participates in admissions, discharges, and transfers of residents according to organizational standardCollaboration and Communication:Participates in collaborative practice with all members of the care team.Participates with the interdisciplinary team, resident, and family in the development, modification and evaluation of individualized care plans that support the abilities, preferences, and routines of the resident.Communicates with physician(s) regarding assigned resident and carries out verbal and/or written orders as required. Transcribes orders onto appropriate resident record and processes written orders as needed.Identifies resident and their significant other(s)’s actual and potential strengths and limitations including physiological, psychological, socio-cultural, and spiritual needs. Encourages involvement of patient/resident and their significant other(s) in decisions that affect them, by respecting personal choices and providing information and opportunities to talk about their concerns.Uses knowledge of resident status, external supports, the Home for us Model of Care, and other relevant factors to assist the team with decision makingParticipates in staff meetings, special projects, and relevant care home committees to provide generalized information and feedback on resident care and nursing practice.Participates in continuous quality improvement and program evaluation activities, education sessions, team conferences, meetings and committees.Household Maintenance:Encourages resident participation in activities of daily living, and activities of daily life according to the abilities, preferences, and routines of the resident.Responds to a variety of incidents such as medical emergencies, injuries or falls, behavior that may pose a risk of harm to a resident or other residents, and fire alarms. Reports incidents and fills out appropriate forms in accordance with established procedures.Operates and/or cleans equipment related to resident’s care and reports malfunctions to supervisor. Uses appropriate supplies for care in accordance with departmental procedures; handles, stores and disposes of workplace hazardous materials and sharps in accordance with established procedures.Performs related clerical duties such as answering the telephone, faxing, taking, and relaying messages and assisting in maintaining standardized records.Works with the household team to establish daily priorities and sets the order of the day.Provides support and assistance to the household team with light housekeeping, and dietary duties for residents as needed. Assists the household team to ensure that resident routines and preferences are prioritizedAssists the household team with tasks to ensure the household is running smoothlyWorks with the household team to ensure that the environment of the household is prepared in a way that optimizes resident function and a day well lived. This includes some assistance with set-up, tidying, and take-down.Performs other related duties as assigned.
