Summary of Duties:

Supports the philosophy of Person-Centred Care as well as the principles, mission and values of Sunnybrook Health Sciences Centre
-
Reporting to the Patient Care Manager of the Schulich Ambulatory, the Cardiac Care Coordinator is responsible for facilitating the timely access to cardiac services, including:
-
Triage and booking of all referrals related to Cath Lab activity
-
Types of patients triage: Cath, PCI, Cardiac Surgery, Arrhythmia and Structural heart
-
Liaison with patients and referring physicians regarding procedures
-
Wait list management
-
Patient surveillance, history and physical assessment
-
Data collection and analysis
-
Patient and family support and education
-
Participates as member of the Cardiac Care team
-
Participates in CorHealth Ontario activities
-
Bed flow management within Cardiac Cath Lab and Outpatient areas
-
Contribute to policy and procedure development for the arrhythmia clinic
-
Participate in best practices, accreditation readiness and quality improvement initiatives
Qualifications/Skills
- Baccalaureate Degree in Nursing required
- Current College of Nurses of Ontario Certificate of competence
- Current BCLS
- Minimum 5 years’ experience in cardiology required
- Previous Cath Lab experience preferred
- Previous patient education experience preferred
- Statistical data management and quality assurance
- Ability to integrate concept of patient-focused care into clinical practice
- Computer experience essential
- Excellent clinical assessment and critical thinking skills
- Demonstrated satisfactory job performance and attendance
- Ability to adapt to a fast-changing and stressful environment in a positive manner
- Demonstrated effective ability in: interpersonal relationships; oral and written communication skills; priority setting skills; problem solving; decision making; facilitating team work; conflict resolution & time management
- Commitment to the quality care of our patients, evidence based practice and a clear understanding of professional standards
- Ability to integrate and apply critical thinking skills to patient care within scope of practice
- Ability to function independently and as a part of the interprofessional team
