Job Description: As a member of the Primary Health Care Team this individual

will use the customer-centered approach to facilitate behavioral health changes through screening, assessments, brief interventions and education. This individual will hear the customers voice and assist them in achieving their health needs through the development of strong relationships and individualized holistic care plans. Working in a multidisciplinary environment including, but not limited to, Primary Health Care Team members: Family Physicians, Nurse Practitioners, Primary Health Care Case Manager, Home Care, Emergency Medical Services, Pharmacy, Therapy Services, Public Health, Dieticians, Psychiatrists, Psychologists, Social Workers, Addiction Workers, and visiting specialists as this position requires networking with other professionals and agencies within the community. "Preference for this position will be given to HSAS members in accordance with the terms and conditions of the SAHO/HSAS Collective Agreement"

Experience

  • Experience with clinical interventions related to Primary Health Care
  • Experience w/ clinical interventions in Integrated Care or Community Health Care
  • Experience in brief treatment modalities in PHC setting is an asset

Education

  • Degree in Human Services (Social Work, Psychology, Registered Nurse)

  • Mental Health Act, Child and Family Services Act, Health Care Directive
  • Ability to work independently and as a member of a multi-disciplinary team
  • Adherence to the Code of Ethics.
  • Skilled in Motivational Interviewing and mindfulness practices
  • Knowledge of Health Information Protection Act and other legislation.

  • A valid driver's license and a vehicle is required to carry out duties
  • Registered/eligible for registration with the appropriate licensing body
