We are hiring for a Senior Support Coordinator for the Health Centre in our Mastrs & Oasis Program and are looking for you. You will be an integral part of a collaborative, inter-disciplinary team focused on delivering seamless programming and service excellence to our patients.

Hours of work: Permanent Full Time, 35 hours per week

The overarching goal of the Senior Support Coordinator is to provide appropriate, empathetic, non-judgmental and respectful care for persons with serious mental illness and concurrent disorders while ultimately helping the client to increase their quality of life and decrease the time spent in hospital. What you’ll be doing:

  • Provide support coordination for evidence-based, complexities within the senior population with serious mental health and concurrent disorders
  • Conduct comprehensive patient assessments
  • Provide support coordination within a recovery framework to include treatment and community supports with both formal and informal networks
  • Ensure clients are supported in community environments, with engagement in community locations at a minimum of 75% of the service period

What we’re looking for:

  • Bachelors in Psychology, Social Work or other related field
  • Valid registration with the relevant college/governing body in Ontario
  • Knowledge of older adult population needs and concurrent mental health disorders
  • 1 - 3 years’ experience in concurrent mental health and knowledge of the complexities within the senior population
  • Valid driver’s license, business insurance and personal vehicle
