DESCRIPTION:
The Registered Nurse provides services in a manner consistent with KHSC Mission, Vision and Values and that reflects the core values of Patient- and Family-Centred Care (dignity, respect, information sharing, participation, and collaboration); practices in accordance with the standards of professional practice and ethical values as defined by the College of Nurses of Ontario; works as a member of a multidisciplinary team in the Heads Up! Early Psychosis Intervention Program, a regional specialized outpatient mental health service that responds to the early intervention needs of 14 to 34 year old clients in Southeastern Ontario; uses advanced skills in assessment, treatment and psycho-education to deliver innovative client-centred services to individuals and families who are experiencing a first episode of psychosis; works in a program that uses an interdisciplinary approach to provide clinical services to individuals, couples, families and groups in both our Central District which includes locations across Frontenac, Lennox & Addington Counties and our Belleville office which includes locations across Hastings and Prince Edward Counties; this position is based both in the Kingston office (HDH Site) and in the Belleville office (community location); provides skilled services at an advanced level of practice including medical services (e.g. injections) in client’s homes or local pharmacies; works in close collaboration and partnership with other mental health and addiction providers in the Southeast region; utilizes an effective scope of clinical practice for direct service, consultation, program development, teaching and evaluation; includes collaborative interdisciplinary strategies in their professional practice that promotes effective problem solving; demonstrates expertise in public speaking and participates in the planning, presenting and evaluating of psycho-educational information and programs; collaborates and communicates effectively with clients, families and all members of the health care team of multidisciplinary professionals and support personnel; participates in service and hospital wide activities and programs that model and promote a safe and healthy patient-centred environment and that contribute to quality improvement and risk management initiatives.
PRIMARY RESPONSIBILITIES & DUTIES:

Initiates and coordinates interprofessional team assessments and plans of care including physical, spiritual and psychosocial needs of patients across the continuum of care.

Delivers care and provides care coordination for an assigned group of patients in an ambulatory and/or inpatient setting.
-
Plans, organizes and establishes priorities based on individual patient’s needs and scheduled activities and adjusts priorities and activities in response to unanticipated events.
-
Develops plan of care to achieve identified patient goals within the ambulatory and/or inpatient setting and including planning for transitions within and from the hospital setting.
-
Provides teaching and learning experiences for both patient and family.
-
Coordinates family communication, family meetings and conferences.
-
Performs therapeutic measures as prescribed by members of the professional care team and as indicated by the nursing diagnosis.

Maintains appropriate records of observation, nursing care administered and patient’s response.
-
Communicates regularly with the interprofessional team members throughout the patient care experience.

Assigns, teaches and supervises Patient Care Assistants/Volunteers.

Acts as the key contact person for new interprofessional team members and assigned group of patients.

Performs Charge Nurse responsibilities for a tour of duty as assigned.

Provides for an environment which is conducive to the safety and well-being of patients, visitors and staff by practicing and promoting accident prevention; by being familiar with emergency procedures and regulations; by using skills, physical facilities, supplies and equipment safely and appropriately; and by being alert to infection hazards and infection control procedures.

Maintains competence in nursing practice through in-service and continuing education.

Participates in nursing research.

Serves on departmental and related committees when and as requested.

Interprets nursing department and hospital policies to patients, families and visitors.

BASIC QUALIFICATIONS:

Eligible for registration with the College of Nurses of Ontario.

Experience working in an Early Psychosis Intervention Program (preferred)
-
One year recent related experience (preferred)

You have demonstrated ability to intervene effectively in clinical situations requiring the management of aggressive behaviour.
-
Strong knowledge of, and ability to apply principles of crisis intervention; and an understanding of psychiatric nursing as appropriate for this patient population is required.

Highly developed clinical skills in interviewing, psycho-social and risk assessment, treatment (individual, group and family therapy) crisis intervention, motivational interviewing and cognitive behaviour therapy (CBT) with patients and families

BCLS Certificate @ Health Care Provider’s Level
-
2 years’ experience in mental health nursing
-
Must have an automobile, a valid driver’s licence and necessary insurance to be able to travel in the Southeastern Ontario Region on a regular basis, and transport clients when necessary

In-depth knowledge of Ontario Mental Health Act, Health Care Consent Act, Powers of Attorney Act, Substitute Decisions Act and other relevant legislation

Demonstrate knowledge, skill and ability in dealing effectively with patients with severe mental illness.

Demonstrated knowledge of DSM-V TR diagnostic criteria and psychopharmacological –somatic therapies, community based and regional mental health and other relevant resources
-
Proven effective interpersonal and communication skills

Demonstrated ability to effectively conduct a needs assessment, develop collaborative care plans, educational tools and programs and facilitate individuals learning experiences for patients
-
Demonstrated ability to effectively deal with conflict situations and proactively and effectively resolve issues impacting on quality and continuity of care

Excellent time management, organizational and prioritization skills

Proficiency with Microsoft Office applications including Outlook, Word, Excel

Capable of coping in a physically and mentally demanding work environment
