If you are passionate about helping people and you like to see how the work you do makes a difference to people on a daily basis, working at Bridges Community Health Centre (CHC) will provide you with the opportunity to do meaningful work, with like-minded healthcare providers, providing care for those who experience barriers and the effects of the social determinants of health. As one of a network of provincial community health centres, Bridges CHC is a community based, non-profit organization funded by Ontario Health, emphasizing access to primary health care and illness prevention programs and services.
AS A MEMBER OF OUR INTERDISCIPLINARY TEAM, THE NURSE PRACTITIONER
- functions independently but works alongside other Centre staff to provide client-centered care that emphasizes health promotion, disease prevention/treatment and client education.
- The nurse practitioner provides primary health care, within the PHC NP scope of practice, to clients at all stages of their lives and with all health conditions.
- The nurse practitioner practices according to the Standards of Practice of the College of Nurses of Ontario (CNO) and guidelines and policies established at Bridges Community Health Centre.
- The nurse practitioner works in partnership with other staff and management to support the Vision, Mission, and Values of the Centre.
QUALIFICATIONS INCLUDE
- Qualified to register or be registered in good standing with the College of Nurses of Ontario as a Registered Nurse in Extended Class.
- Bachelor or Master of Science in Nursing from a recognized university.
- Three to five years’ experience in a health care setting with preferred experience in the community.
- Demonstrated ability, commitment to and knowledge of community health care, the social determinants of health and health promotion.
- Demonstrated commitment to equity, diversity, inclusion, and anti-racism with a willingness to engage in ongoing learning to increase self-awareness, improve practices and eliminate disparities in health status.
- Awareness of the importance of, and experience with, providing trauma-informed care.
- Excellent interpersonal skills with clients in a culturally diverse practice population, and with colleagues.
- Demonstrated ability to work effectively in an interdisciplinary team environment.
- Excellent communication, decision-making, problem-solving, conflict management and time management skills.
- The ability to work at either Bridges CHC location and work flexible hours.
- Participation in our afterhours, on-call rotation schedule.

Bridges Community Health Centre serves the communities of Fort Erie, Port Colborne and Wainfleet with a focus on the needs of seniors, children and youth, newcomers to Canada, and families or individuals with low incomes.
The staff at Bridges CHC includes physicians, nurse practitioners, nurses, dietitians, diabetes educators, social workers, psychotherapist, health promoters, community health workers, physiotherapists, kinesiologist and physiotherapist assistant.
Our primary care team, support clients of Bridges CHC and our health promotion programs are available at no cost and open to all members of the community who are looking to improve their health. To promote healthy living in our communities, our programs and peer support groups can help you with smoking cessation, diabetes education, mental health, chronic pain management, stress management, exercise and healthy eating.
