The Care Company is looking for an awesome, self-driven, highly organized and detail-oriented Nurse (RN or RPN) to assist us with complex care paediatric in the west end of Toronto. There is certain criteria in place to care for this client. Please read below and only apply if you meet all of the criteria:
- Provide nursing care with experience in G-tube management, catheter care, and seizure monitoring/management.
- Perform safe and independent client transfers (client weighs approximately 35–40 lbs) between bed and chair.

- No lifting up or down stairs is required.
- Frequent lifts are part of the role; candidates with back issues or physical limitations may not be a suitable fit.
- Maintain a flexible approach to care, following directions where safe and within scope of practice.
- Establish and maintain professional boundaries
- Demonstrate resilience and professionalism, with the ability to accept feedback without taking it personally, as the family has specific care expectations.
- Accompany client to and from medical appointments when required.
Scheduling/Availability Requirements:
This client requires care in the EVENING between 1pm-10pm on Tuesdays, Wednesdays, and Thursdays. Please only apply if you can commit to an ongoing, consistent, weekly schedule of evening shifts.
**PLEASE NOTE this client is looking for a nurse to join their team on a long-term basis as they invest a lot of time and energy in training and onboarding new staff.**
Requirements
The following Requirements are MANDATORY:

Active registration and in good standing with the College of Nurses of Ontario (CNO) as a Registered Nurse (RN)
-
Current CPR and First Aid certification, completed through a recognized training provider within the past 12 months.
-
Valid WHMIS 2015 certification, issued or renewed within the past 12 months.
-
Negative TB test (2-step or 1-step with prior documentation), completed within the past 12 months.
-
Proof of full COVID-19 vaccination, or exemption in accordance with the Ontario Human Rights Code.
-
Clear Criminal Record Check with Vulnerable Sector Screening, issued within the past 12 months.
-
Must possess a valid Ontario driver’s licence and access to a reliable vehicle for travel to client homes and schools across the Greater Toronto Area (GTA).
-
Must be able to communicate effectively in English, both orally and in writing, for documentation and communication with clients and care teams.
-
Demonstrated experience delivering paediatric care.
Must have hands-on clinical experience providing:
- Gastrostomy (G-Tube) feeding and care for pediatric clients
- Catheter care for paediatric clients
- Seizure monitoring and management protocols for pediatric clients with known seizure disorders
- Ability to identify, document, and escalate any changes in condition to the supervising nurse or care coordinator.
Duties and Responsibilities:
- Provide one-on-one personal care to clients in their home environment, including full assistance with bathing (bed baths, showers, perineal care), oral hygiene, grooming, toileting, incontinence care, dressing, and feeding, in accordance with the client’s care plan and scope of practice for a Registered Nurse (RN)
- Administer medications orally, topically, subcutaneously, or via G-tube, as per CNO scope and documented medical orders; document all administrations in the electronic Medication Administration Record (eMAR).
- Monitor and assess clients’ vital signs (blood pressure, pulse, temperature, respirations, oxygen saturation), document findings, and report any abnormal readings or concerns to the designated Care Coordinator or physician.
- Support clients with medical needs such as oxygen therapy (monitoring levels, changing tanks, checking saturation), G-tube feeding (preparing and administering formula, flushing lines), and suctioning as per training and client-specific protocols.
- Safely assist clients with mobility and transfers using mechanical lifts (Hoyer, sit-to-stand) or manual assistance, following prescribed mobility plans and ensuring proper body mechanics and safety procedures.
- Perform prescribed range of motion (ROM) and physical therapy exercises under direction of a physiotherapist or care plan, and assist with ambulation inside the home or outdoors if required.
- Observe and document any changes in the client’s physical, cognitive, or emotional condition, including changes in skin integrity, alertness, behaviour, and breathing patterns; escalate concerns as needed.
- Maintain accurate and timely documentation of all care provided, including progress notes, incident reports, and communication logs, in compliance with company and regulatory standards.
- Use and dispose of Personal Protective Equipment (PPE) such as gloves, masks, gowns, and eye protection as per infection prevention and control protocols and Public Health Ontario guidelines.
- Comply with organizational policies regarding infection control, client safety, privacy, and professional conduct; report all hazards or incidents to the supervisor immediately.
