About the role
The purpose of this Registered Dietitian position is to maintain or improve the health status of clients of the PMH by participating as an integral member of the team to assist individuals, families and communities in promoting and maintaining health and preventing illness. This position is clinically supervised by the Primary Care Network Clinical Lead and reports to the Network Manager.
The primary role of this position is to carry out independent visits with clients of the health centre/Network in collaboration with a health centre team to assess, plan and implement care and to monitor and provide follow-up of chronic diseases. This position will develop and deliver health education sessions on an individual and group basis to clients, health professionals and the public.
Duties will include: Assessing appropriate aspects of the client’s plan, manage and monitor the achievement and maintain optimal health for the client; Providing individual and group education on Chronic Disease to clients in accordance with provincial policies and national standards in Canada; Collaborating with team members, primary care physicians/nurse practitioners and others (specialist physicians and other health care professionals) to review, recommend and monitor chronic disease management protocols. Maintaining accurate client records, reports and communication related to nutrition assessment for dissemination to team members, primary care physician/ nurse practitioner and physician specialist following referrals. Working to develop appropriate educational materials, presentations and information sheets for use with individuals, groups and the general public. Participating in the development of program standards and services, procedures and protocols affecting the operation of the Primary Care Network Program; Maintaining a current knowledge base regarding trends in chronic disease management via workshop attendance and reading publications. Performing other related duties.
Minimum Qualifications: Must have a completed Bachelors degree with a major in Nutritional Science or equivalent from an approved academic institution. Must have completed a Dietetic Internship program accredited by PDEP (Partnership for Dietetic Education and Practice). Applicants must be registered with the College of Dietitians of Prince Edward Island and be eligible for membership with Dietitians of Canada. Must have experience as a practicing Registered Dietitian. Demonstrated ability to make recommendations based on current evidence-based practice. Excellent written and verbal communication skills and interpersonal skills. Proficient computer skills, such as word processing, spreadsheets, presentation software, and internet. Possess a valid driver’s license and access to a reliable vehicle. Excellent previous work and attendance record.
Other Qualifications: Demonstrated ability to work independently within professional expertise and apply sound clinical decision making. Demonstrated ability to work cooperatively and collaboratively in an interdisciplinary team environment. Strong communication skills, both written and verbal. Effective time management and organizational skills. Demonstrated commitment to continuous professional development. Proficiency in a computerized environment. Ability to develop and lead training sessions for clients. Demonstrated leadership, interpersonal, facilitation, critical thinking, clinical reasoning, and collaboration skills. Knowledge and understand evidence-based practice. Knowledge of CHR, CIS, Outlook, and Microsoft Office programs will be considered an asset. Additional relevant education and training will be considered an asset.
Salary Range: $36.20 - $45.26 per hour (Level 17A – UPSE Health) Bi-Weekly Hours: 75 Hours Bi-Weekly / Days, Evenings and Weekends.
