At present, our wide array of services delivers much needed support to over 4,000 people/year who live with mental health challenges, psychogeriatric mental health conditions, acquired brain injuries, developmental disabilities, dual diagnoses and/or the experience of homelessness. Role Overview
The Registered Nurse, Geriatric Mental Health Case Manager provides case management support and care for the over all well-being of service users. The Registered Nurse, Geriatric Mental Health Case Manager will provide support to service users in their homes and community settings in North York Region.
About the Team:
The NY-IST Team is an interdisciplinary team of professionals that supports people with an Acquired Brain Injury, Developmental Delay, Mental Health and Psycho-geriatric service users as well as their families. The team is composed of professionals who work as one team to support the complex healthcare needs of this service user population, their families and caregivers.
Primary Responsibilities:
The Registered Nurse, Geriatric Mental Health Case Manager's primary duties may involve, but not necessarily be limited to:
- Work collaboratively with internal and external partner agencies to provide short term CM care for service users of 65 years old and older with some exceptions
- Network with external agencies to obtain relevant resources for service users' care and to make referrals to Retirement Homes and Long Term Care facilities as needed
- Support/encourage service users to participate in community activities to build social network
- Advocate with and on behalf of service users to improve and/ or maintain wellbeing
- Create, maintain and improve healthy working relationships with service users and their circle of care
- Work collaboratively with their team members to support all service users on the North York Integrated Service Team
- Attend and engage with others on the Cota Geriatric Community of Practice meetings and network with other Geriatric case managers
- Be accountable for meeting benchmarks and completing documentations on time
- Contribute to the overall growth and wellbeing of the Team
- Keep up with relevant knowledge and information to better support the service users
Qualifications:
Education & Experience:
Either:
- A Bachelor’s degree in Nursing (must be Registered with the CNO, College of Nurses of Ontario)
- One year of experience in a position working with the Geriatric Population
OR,
- Other relevant education and experiences that is determined to be acceptable by the interviewers
Knowledge & Skills:
- Knowledge of Dementia and other age- related illnesses is preferred
- Knowledge of Diversity Equity and Inclusion issues and Intersectionality
- Ability to model Cota’s core values with clients, co-workers and partner agencies
- Knowledge and understanding of the client population and an ability to work effectively with people living with serious mental illness respectfully
- Demonstrated strong commitment to the rights and ability of each person with a serious mental illness to live successfully in community residences, work in market jobs, and have access to helpful, adequate, competent, and continuous supports and services
- Knowledge of principles of case management theory and practice using a client-centred and mental health Recovery philosophy
- Excellent clinical judgement and decision-making skills including a keen awareness of service users-clinician boundaries and ethical dilemmas
- Experience working on a multidisciplinary team is an asset
- Experience working in a community agency is an asset
- Experience working with external organizations and building positive working relationship
- PIECES certificate training is an asset
- Knowledge of Microsoft Office Suite (Word, Excel, etc.)
- Excellent knowledge of community resources and/or demonstrated ability to search for applicable resources/information
Other:
- Visits will be conducted in the service users’ homes, clinics, hospital, shelters, and other community settings as required and requires staff to travel-by vehicle drive to get there
- Role adjustment as needed
- Access to a vehicle (Please note that access to a vehicle means owning, borrowing, or renting/hiring a vehicle at no cost to Cota.)
Benefits:
Upon successful completion of a three-month period of employment, permanent full-time and permanent part-time staff working more than 20 hours per week as well as temporary full-time (with a contract for at least 1 year) and temporary part-time staff (with a contract for at least 1 year and working more than 20 hours per week) will be eligible for employer-paid group insurance benefits. These employer-paid benefits will include:
