HOURS OF WORK:
The stated hours of work are approximate as schedules are subject to change with the operational needs of the Health Centre. Changes made to hours of work will follow the terms of the applicable collective agreement, company policy and/or legislation.
WHAT YOU’LL DO:
The Peritoneal Dialysis (PD) Nurse as one member of the healthcare team has a unique role in promoting health and helping patients attain and maintain the highest level of health possible. S/he is responsible for providing comprehensive care to patients, including training patients to conduct their own dialysis treatments and monitoring on-going care. The PD nurse will also provide on-going support and education to inpatient and home care nurses and required. Patients may have predictable and unpredictable outcomes who may or may not be clinically stable. The Peritoneal Dialysis Nurse practices in accordance with the College of Nurses of Ontario Professional Standards.
WHAT YOU’LL BRING:
- Current certificate of competence from the College of Nurses of Ontario.
- CNA certification – C Neph (C )
- Current BCLS certification required.
- Valid G driver’s license and access to a reliable vehicle
- Outpatient Home Peritoneal Dialysis experience within last 2 years preferred.
WHO YOU ARE:
- Highly developed physical assessment skills
- Excellent knowledge of the renal system pathophysiology
- Expert knowledge and experience in adult education and health teaching skills
- Demonstrated ability to exercise good clinical judgement
- Demonstrated effective time management
- Ability to create patient education material
- Ability to work in fast paced clinical setting
- Demonstrated good communication skills including evidence of strong documentation skills
- Must have the ability to work well independently and as a member of a multidisciplinary team
- Demonstrated ability to work well with patients, families, and all levels of staff in the community
- Demonstrated commitment to ongoing education
- Demonstrated ability to provide education to patients and family members following the principles of adult learning
- Excellent knowledge of the principles of motivational interviewing
- Basic computer and keyboarding skills
- Willingness to adapt to new technology
- Demonstrated ability to maintain a good attendance record
- Demonstrated ability to conduct internet research
- Experience with Epic would be an asset
- Experience using Microsoft 365 products such as outlook, excel, and word considered an asset
