Job Description

The Personal Support Worker provides for the individual needs of the residents according to established resident care plan, all components of activities of daily living and supporting participation in recreational and therapeutic activities.

Ensures and promotes the provision of resident-focused care to meet the needs of residents as follows: Promotes the philosophy and practices of resident centeredness in all residents’ encounters and decisions. Promotes the Residents’ Bill of Rights.

Supports and helps provide a Resident Advocacy Program

Maintains a wellness and rehabilitative perspective. Facilitates relationships between and among residents, relatives, team member and community. Supports opportunities for resident/ family participation in decisions that affect the resident’s rights and lifestyle.

Provides supervision and/or assistance to the residents in the activities of daily living including eating, dressing, transferring, toileting, hygiene, grooming and bathing as per the residents’ individual plan of care to meet their needs and expectations as follows: Improves the function in the specific personal care area Provides rehabilitative care processes Maintains the highest level of functioning Promotes independence and individual preferences Observes and reports any changes in the resident Records on flow sheets and other documentation as required

Provides care and services to the residents with respect to their senses and communication by encouraging residents to wear individual sensory aids such as glasses, hearing aides, etc.

Provides cognitive, social, emotional and spiritual support as follows

Seeks direction/assistance implementing orientation plan for an impaired resident Encourages resident in decision making Implements care plan to prevent/manage responsive behaviours and other challenging behaviours Observes and respects resident’s rights, dignity and privacy Provides oral and dental care as follows: Provides individual mouth and dental care to residents as indicated in the care plan Provides labeled dentures to each resident if applicable at morning care and removes at bedtime and stores appropriately Observes for mouth, tongue or dental problems, in particular breath odour

Provides for and implements preventive and safety measures and reports unsafe conditions such as the removal of “sharps” from resident’s room, foot rests, and side rails, as per care plan and notifies the housekeeping department of broken glass, china, and remove spills on floors. Provides for and assists in achieving the residents’ nutritional needs as follows: Prepares and assists residents to and from dining room as requested or required.

Assists residents in choosing their food preferences

Assists and supervises residents with eating at scheduled meals, teas and snacks Assists and instructs residents in the use of self-care feeding devices Reports and records significant observations (e.g. food and fluid intake, hydration)

Determines change in skin integrity and implementation of skin care to prevent skin breakdown as ordered by the registered team member. Provides for and assists with resident’s elimination needs as determined in the individual care plan as follows: Administers and removes bed pans and urinals Assists residents to and from commode chairs or toilets as per policy Follows bowel and bladder training program and incontinent care Provides routine personal care and bathing as scheduled by inspecting each resident’s feet and nails and reports concerns to the registered team member Provides comfort, rest and sleep to residents in their physical environment, positioning, and health treatment and care routines as per care plan.

Provides support services to meet the needs of the residents as follows

Porters residents to various appointments Uses sanitation and infection control procedures Cleans hydrotherapy units, tubs, shower chairs and lift chairs with a germicidal cleaner as per policy and procedure Promptly identifies and addresses incidents of offensive odour Follows directions of registered team member in an emergency situation

Assists in financial management by adhering to the home’s policy and procedures as directed by DOC regarding finances Participates in material management of the home as follows: Uses supplies for resident care appropriately as directed by Registered team member Reports any equipment malfunctions to Registered team member Assists with inventory control Provides input into selection of equipment or supplies as requested

Participates in human resource management as follows

Follows all human resources policies and procedures Assists with orientation of new team member and volunteers Participates in team member in-service and education sessions outside of the home Participates with student placements within the home

Implements infection control measures

Proper hand hygiene at four points of care Handles soiled linen /clothing as per policy and procedure Cleans/disinfects equipment as per infection control protocols Participates in records management as follows: Documents as indicated in policies and procedures in the resident chart Completes documentation prior to the end of the shift Completes documentation required on any incidents

Participates in the home’s quality management as follows

Completes resident audits as required Participates as requested on the home’s teams/committees Participates in quality improvement initiatives Knows and interprets MOHLTC legislation, regulation and process. Work in accordance with the organization’s Health and Safety Policies and Procedures and in compliance with the Occupational Health and Safety Act. Perform other duties as required.

Required Skills

Successful completion of a Personal Support Worker certificate from an approved college or private institution meeting the standards established by the Ministry of Training, Colleges and Universities, National Association of Career Colleges, or Ontario Community Support Association.
