THE OPPORTUNITY

This individual identifies and addresses care processes and /or other issues impeding the flow of patient care from admission to discharge destination and ensures the provision of safe, quality care and safe transitions for patients at GBGH. These individuals act as a resource and role model for all clinical staff and works collaboratively with the care teams to provide leadership congruent with the vision, mission, values and strategic objectives of the hospital.

KEY ACCOUNTABILITIES & RESPONSIBILITIES

  • In collaboration with the health care team, conduct patient navigation activities to meet or exceed Georgian Bay General Hospital target ED Wait Times, facilitate and coordinate consultations, treatments, tests, procedures and address any delays or interruptions.
  • Review all potential ALC patients and in conjunction with CCAC and the interdisciplinary team identifies strategies to avoid ALC designation.
  • Coordinate navigation activities and information handover with the team to ensure optimal hospital wide patient flow minimizing admission, discharge and transfer peaks and pressures and departmental functional processes.
  • Facilitate interdepartmental, inter-hospital and interprofessional communication amongst the team and with patients and families.
  • Support documentation, monitoring and communication.
  • Participate actively in continuous quality improvement initiatives.

QUALIFICATIONS & EXPERIENCE The ideal candidate will possess the following key competencies, record of experience and academic preparation:

  • Registered Nurse with current certificate of registration in good standing with the College of Nurses of Ontario
  • Bachelor’s Degree or Health Care degree an asset
  • Five (5) years acute care nursing experience, assessment skills and demonstrated clinical proficiency in all aspects of nursing required. Current and/recent practice considered an asset
  • Proven success in coordination of patient plan of care and related activities in the presence of conflicting information and proven priorities; demonstrated ability in conflict management and resolution, required
  • Discharge planning experience preferred.
  • Quality improvement experience preferred; Knowledge and able to utilize outcome measures
  • Demonstrated ability to set priorities and effectively resolve challenging situations that involve multiple stakeholders, patients and families
  • Excellent interpersonal and leadership skills required, with a high level of verbal and written communication skills
  • Effective mediation/negotiation skills combined with the ability to work in collaboration with all members of the health care team both internally and externally across the continuum of care
  • Ability to be self-directed and thrive in a dynamic environment
  • Excellent clinical, decision making and critical thinking skills
  • Ability to adapt to change in a fast-paced working environment
  • Demonstrated consistent punctuality and attendance record, maintaining a high level of reliability and commitment to performing duties with efficiency and effectiveness in previous roles

  • As a partially designated French speaking organization, French verbal and written communication skills are considered an asset in this position

  • Hourly rate of pay $41.30 - $59.19, vacation accrual starts at three (3) weeks per year

HOURS OF WORK Full Time. Must be available to work shifts including days, evenings, nights and weekends. Shifts and hours may change according to departmental requirements and as per the ONA collective agreement.
