Job Details This position is responsible for helping patients and their families to navigate and access community services and other resources, and for supporting and maintaining population health programs and care coordination activities. The Case Management Extender works with Partners for Kids staff, hospital staff, health plans, and community agencies to arrange for and/or connect patients to needed services. The Case Management Extender will identify, create and nurture relationships with local agencies, schools, churches and other programs that can provide services to children and their families. Care Coordination Extenders are patient and family facing and they interact with them on a daily basis. These activities are performed in a variety of settings including during inpatient stays, outpatient specialty clinic visits, emergency room visits, home and community visits and telephonically.
